盐城市大丰区农村老年人签约家庭医生状况及其影响因素

Status of Signing a Contract with Family Doctor Among the Rural Elderly in Dafeng District of Yancheng City and Its Influencing Factors

目的:了解大丰区农村老年人签约家庭医生的状况及其影响因素,为完善老年人家庭医生签约服务提供参考。方法:采用分层整群抽样法在大丰区抽取1600名农村老年人进行问卷调查,调查老年人的一般情况和签约状况,运用χ^(2)检验和logistic回归分析签约家庭医生的影响因素。结果:共得到1480份有效问卷,38.8%的老年人知晓家庭医生签约服务,26.0%的老年人已经签约家庭医生服务。多因素分析显示知晓途径为政府宣传(OR=10.426),经常服药(OR=8.461),收入不够用(OR=3.950),患慢性病(OR=2.994),健康自评差(OR=2.802)的老年人签约率高。结论:大丰区农村老年人对家庭医生签约服务的知晓率及签约率不高,应加大政府宣传,提高签约服务质量;关注低收入及健康状况较差的老年人,优化家庭医生服务包,发挥家庭医生签约服务的作用。

Objective:To study the status and influencing factors of signing a contract with family doctor among the rural elderly in Dafeng district,so as to provide a theoretical basis and practical reference for the elderly family doctor contracting services.Methods:A stratified cluster sampling method was used to select 1600 rural elderly people in Dafeng to do a questionnaire survey,to investigate the general situation and the status of contracting service of the elderly.Chi-square test and logistic regression were used to analyze the influencing factors of signing a contract with the family doctor.Results:A total of 1480 valid questionnaires were obtained.38.8%of the elderly had heard of family doctor contracting services,26.0%of the elderly had signed contracts with family doctors.Multivariate analysis results showed that the elderly with government propaganda(OR=10.426),long-term medication(OR=8.461),insufficient income(OR=3.950),chronic diseases(OR=2.994)and poor health condition(OR=2.802)had high signing rate.Conclusion:Both the awareness rate of family doctor services and the rate of signing a contract with family doctor are not high among rural elderly in Dafeng district.It is necessary to increase government publicity and improve the quality of contracting services,pay attention to the elderly with low income and poor health condition,optimize family doctors'contract service packages,give full play to the role of contracting services of family doctors.
