脑梗死并发肺部感染炎症细胞因子基因多态性 被引量:4

Gene polymorphisms of inflammatory cytokines in cerebral infarction patients complicated with pulmonary infection

摘要 目的分析脑梗死并发肺部感染炎症细胞因子基因多态性。方法选取2018年1月-2020年5月在南阳市中心医院住院的脑梗死患者180例,根据是否发生肺部感染分为肺部感染组(n=50例)和无肺部感染组(n=130例)。采用酶联免疫吸附法测定白细胞介素-6(IL-6)浓度,采用限制性片段长度多态性聚合酶链反应技术分析IL-6基因-572 C/G基因多态性,分析其与肺部感染的相关性。结果肺部感染组的血清IL-6、白细胞计数(WBC)、血小板(PLT)水平高于无肺部感染组(P<0.05),两组谷丙转氨酶(ALT)、尿素氮(BUN)、血肌酐(Cr)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)水平比较,无统计学差异;肺部感染组基因型GG频率高于无肺部感染组(P<0.05),两组等位基因频率、基因型GC、CC频率比较,无统计学差异。GG基因型患者的IL-6、WBC水平高于CC+CG基因型患者(P<0.05);Logistic回归分析结果显示,年龄>60岁、有侵入性操作、合并基础疾病、既往肺部疾病、IL-6-572 C/G为影响肺部感染的危险因素(P<0.05)。结论IL-6基因-572 C/G的多态性可能与脑梗死患者肺部感染有关,基因G可能是易感基因,基因型GG可能是易感基因型,CC基因型是保护基因型。 OBJECTIVE To analyze the gene polymorphisms of inflammatory cytokines in the cerebral infarction patients complicated with pulmonary infection.METHODS A total of 180 cerebral infarction patients who were hospitalized in Nanyang Central Hospital from Jan 2018 to May 2020 were enrolled in the study and divided into the pulmonary infection group with 50 cases and the non-pulmonary infection group with 130 cases according to the status of pulmonary infection.The concentration of interleukin-6(IL-6)was determined by enzyme-linked immunosorbent assay,the IL-6-572 C/G gene polymorphism was analyzed by polymerase chain reaction-restriction fragment length polymorphism,and its correlation with lung infection was observed.RESULTS The levels of serum IL-6,white blood cell(WBC)and platelet(PLT)of the pulmonary infection group were significantly higher than those of the non-pulmonary infection group(P<0.05).There were no significant differences in the levels of alanine aminotransferase(ALT),urea nitrogen(BUN),serum creatinine(Cr),total cholesterol(TC),triglyceride(TG)and high density lipoprotein cholesterol(HDL-C)between the two groups.The frequency of genotype GG of the pulmonary infection group was significantly higher than that of the non-pulmonary infection group(P<0.05);there were no significant differences in the frequency of allele and frequencies of genotypes GC and CC.The levels of IL-6 and WBC of the patients with genotype GG were significantly higher than those of the patients with genotype CC+CG(P<0.05).Logistic regression analysis showed that the more than 60 years of age,invasive procedures,complication with underlying diseases,previous history of pulmonary disease and IL-6-572 C/G were the risk factors for the pulmonary infection(P<0.05).CONCLUSION The IL-6-572 C/G gene polymorphism may be related to the pulmonary infection in the patients with cerebra infarction.Gene G may be a susceptible gene,genotype GG may be a susceptible genotype,and CC genotype is a protective genotype.
