摘要
目的研究多排螺旋CT(MSCT)联合多平面重组技术对磨玻璃密度结节样肺癌患者的诊断价值。方法回顾性分析大连市中心医院从2018年1月-2020年1月收治的疑似磨玻璃密度结节样肺癌患者112例的病历资料。以手术病理组织活检结果为金标准,比较常规轴位MSCT图像和MSCT联合多平面重组技术诊断磨玻璃密度结节样肺癌的效能。此外,分析两种诊断方式显示磨玻璃密度结节样肺癌征象方面的检出效果。比较不同临床分期磨玻璃密度结节样肺癌患者的MSCT联合多平面重组技术征象。结果以手术病理组织活检结果为金标准,MSCT联合多平面重组技术诊断磨玻璃密度结节样肺癌的灵敏度、特异度、准确度分別为76.60%、67.69%、71.43%,均高于常规MSCT的57.45%、46.15%、50.89%,差异有统计学意义(P值均<0.05)。MSCT联合多平面重组技术对空泡征或支气管充气征的显示率为72.34%,高于常规MSCT的44.68%,差异有统计学意义(P值均〈0.05)。TNM分期为Ⅲ〜Ⅳ期磨玻璃密度结节样肺癌患者空泡征或支气管充气征、边缘分叶征人数占比均高于TNM分期为Ⅰ〜Ⅱ期患者,差异有统计学意义(P值均<0.05)。结论MSCT联合多平面重组技术诊断磨玻璃密度结节样肺癌患者的价值较高,且对空泡征或支气管充气征方面优势尤为明显。
Objective To study the diagnostic value of multi-slice spiral CT(MSCT)combined with multiplane recombination technique in patients with ground-glass nodule lung cancer.Methods The medical records of 112 patients with suspected ground-glass nodule lung cancer admitted to the hospital from January 2018 to January 2020 were retrospectively analyzed.The results of surgical biopsy were used as the gold standard to compare the efficacy of conventional axial MSCT images and MSCT combined with multiplanar recombination in the diagnosis of ground-glass nodule lung cancer.In addition,two diagnostic methods were analyzed to show the detection effect of ground-glass nodule lung cancer.MSCT combined with multiplanar recombination technique were compared in patients with nodular lung cancer with different clinical stages of ground glass density.Results Based on the gold standard of surgical pathological biopsy results,the sensitivity,specificity and accuracy of MSCT combined with multiplanar reconstruction in the diagnosis of ground-glass nodule lung cancer were 76.60%,67.69%and 71.43%,respectively,which were significantly higher than 57.45%,46.15%and 50.89%of conventional MSCT(all P<0.05).The display rate of MSCT combined with multi-plane recombination technology for vacuole sign or air bronchogram sign was 72.34%,which was significantly higher than 44.68%of conventional MSCT(all P<0.05).The percentage of cavitation character or character,air-filled bronchi inⅢ-Ⅳstage ground-glass nodule lung cancer patients were significantly higher thanⅠ-Ⅱstage patients(all P<0.05).Conclusion MSCT combined with multiplanar recombination technique is of high value in the diagnosis of ground-glass nodule lung cancer,and it is particularly advantageous in the cavitation sign or air bronchus sign.
