老年股骨转子间骨折患者临床特征及血液指标分析

An analysis of clinical characteristics and blood indexes of elderly patients with femoral trochanteric fracture

目的分析老年股骨转子间骨折患者临床特征及血液指标。方法依据年龄,老年股骨转子间骨折患者412例分为四组:A组61例,年龄60~69岁,B组149例,70~79岁,C组171例,80~89岁,D组31例,≥90岁。分析各组患者的临床特征、血常规、血生化及凝血五项。结果 D组大粗隆、Ward’s三角和总体骨密度低于A组(P<0.05)。四组患者RBC、Hb、总蛋白和白蛋白均低于正常范围,国际标准化比值、凝血酶原时间和D-D均高于正常范围。A组RBC高于B、C、D组,A组Hb、红细胞比容高于B、D组,A、B组红细胞分布宽度标准差低于C、D组,B组平均红细胞体积低于C、D组(P<0.05)。D组APTT低于B、C组,D组纤维蛋白原高于A、B、C组(P<0.05)。结论股骨转子骨折多发生于70~89岁患者,会出现不同程度的贫血、低蛋白血症及深静脉血栓,以超高龄患者最为严重。

Objective To analyze the clinical characteristics and blood indexes of elderly patients with femoral trochanteric fracture.Methods According to age,412 elderly patients with femoral trochanteric fracture were divided into four groups of A(60-69 years old,61 cases),B(70-79 years old,149 cases),C(80-89 years old,171 cases) and D(≥90 years old,31 cases).The clinical characteristics,blood routine, blood biochemistry and blood coagulation were analyzed.Results The density of greater trochanter,Ward’s triangle and total bone was lower in group D than that in group A(P<0.05).The RBC,Hb,total protein and albumin were lower in four groups than normal ranges.The international standardized ratio,prothrombin time and D-D were higher in four groups than normal ranges.RBC was higher in group A than that in groups of B,C and D(P<0.05).The Hb and erythrocrit were higher in group A than those in groups of B and D(P<0.05).RBC distribution width standard deviation was less in groups of A and B than that in groups of C and D(P<0.05).The mean corpuscular volume was less in group B than that in groups of C and D(P<0.05).The APTT was less in group D than that in groups of B and C(P<0.05).The fibrinogen level was higher in group D than that in groups of A,B and C(P<0.05).Conclusion Femoral trochanteric fracture is mostly seen in the elderly of 70 to 89 years old,which is commonly complicated with different degrees of anemia,hypoproteinemia and deep venous thrombosis,especially in the patients with ultra-high age.
