摘要
目的探讨血栓弹力图(thrombelastogram,TEG)在2型糖尿病(T2DM)合并肺原位腺癌(adenocarcinoma in situ,AIS)患者中的应用及临床意义.方法收集2019年6月—2019年12月40例T2DM合并AIS患者、40例AIS患者及40名健康对照者临床和实验室检查资料,分析比较三组之间TEG参数的差异.结果T2DM合并AIS组患者:(1)R值较AIS组[(3.98±1.47)vs(4.77±1.01),P<0.05]、健康对照组[(3.98±1.47)vs(5.70±1.77),P<0.01]均减小;(2)K值较AIS组[(1.36±0.49)vs(1.73±0.66),P<0.05]、健康对照组[(1.36±0.49)vs(2.26±0.79),P<0.01]均减小;(3)MA值较AIS组[(68.10±8.53)vs(65.48±8.06),P<0.05]、健康对照组[(68.10±8.53)vs(54.82±4.46),P<0.01]均升高;(4)Angle角较AIS组[(70.90±9.57)vs(65.53±9.32),P<0.05]、健康对照组[(70.90±9.57)vs(57.30±11.43),P<0.01]均升高.此外,AIS组R值、K值较健康对照组均减小(P<0.05),MA值、Angle角较健康对照组升高(P<0.01、P<0.05).结论T2DM合并AIS患者血液呈高凝状态.TEG可作为T2DM合并AIS患者凝血功能的精准监测手段之一,对降低术后并发症的发生具有重要的临床意义.
Objective To investigate the appliance of thrombelastogram(TEG)in type 2 diabetes(T2DM)combined with adenocarcinoma in situ(AIS)patients.Methods The clinical and laboratory test data of 40 patients with T2DM combined with AIS,40 patients with AIS and 40 healthy controls were collected from Jun.2019 to Dec.2019,and the differences in TEG parameters between the three groups were analyzed and compared.Results In T2DM combined with AIS group:(1)R value decreased significantly in AIS group[(3.98±1.47)vs(4.77±1.01),P<0.05]and in healthy control group[(3.98±1.47)vs(5.70±1.77),P<0.01];(2)K value decreased significantly in AIS group[(1.36±0.49)vs(1.73±0.66),P<0.05]and in healthy control group[(1.36±0.49)vs(2.26±0.79),P<0.01];(3)MA value increased significantly in AIS group[(68.10±8.53)vs(65.48±8.06),P<0.05]and in healthy control group[(68.10±8.53)vs(54.82±4.46),P<0.01];(4)Angle a value increased significantly in AIS group[(70.90±9.57)vs(65.53±9.32),P<0.05]and in healthy control group[(70.90±9.57)vs(57.30±11.43),P<0.01].Both R value and K value decreased significantly in AIS group than in healthy control group(P<0.05),both MA value and Angle a value increased significantly in AIS group than in healthy control group(P<0.01,P<0.05).Conclusion The blood of patients with T2DM combined with AIS were in hemorheologic levels.TEG can be used as one of the accurate monitoring methods of coagulation function in patients with T2DM combined with AIS,and has important clinical significance in reducing postoperative complications.
