首发抑郁症患者多导睡眠图的对照研究

摘要 目的探讨自然夜间多导睡眠图(PSG)对首发抑郁症患者的诊断效果。方法选取2017年1—12月在该院门诊及住院的首发抑郁症伴失眠患者40例作为实验组,同时选取同期在该院门诊及住院的慢性失眠障碍40例为对照组,分别对两组患者进行自然夜间多导睡眠图诊断,观察两组患者的卧床时间(TIB),睡眠期时间(SPT),睡眠总时间(TST),入睡后觉醒时间(WASO),醒觉次数(AT),醒觉时间(ATA),睡眠潜伏期(SL),睡眠效率(SE),N1、N2、N3、REM睡眠期的结构百分比、REM睡眠时间(RT),REM潜伏期(RL),REM周期数(NRP)情况。结果实验组的SL、SPT明显长于对照组,AT明显少于对照组,SE明显低于对照组,ATA、TIB、WASO、TST明显短于对照组,差异有统计学意义(P<0.05);实验组N2(52.44±0.19)%明显低于对照组(57.77±1.05)%,N1(22.63±0.23)%、N3(18.57±0.28)%明显大于对照组(22.13±0.19)%、(9.26±0.17)%,REM睡眠期的结构百分比(12.13±0.09)%明显小于对照组(12.52±0.03)%,差异有统计学意义(t=31.592、10.600、179.755、26.000,P<0.05);实验组的RT(57.71±0.12)min、RL(148.53±1.26)min明显短于对照组(58.13±0.45)min和(166.79±1.49)min,NRP(3.91±0.01)次明显多于对照组(3.26±0.07)次,差异有统计学意义(t=5.704、59.183、58.138,P<0.05)。结论自然夜间多导睡眠图对于首发抑郁症患者的诊断具有较高的临床价值,能够有效明确患者的TIB、SPT、TST、WASO、AT、ATA、SL、SE、N1、N2、N3、REM睡眠期的结构百分比、RT、RL、NRP情况。 Objective To investigate the diagnostic effect of natural nocturnal polysomnography(PSG)in patients with first-episode depression.Methods Selected 40 patients with first-episode depression and insomnia who were in the outpatient and inpatient departments of the hospital from January to December 2017 as the experimental group,and selected 40 patients with chronic insomnia disorder who were in the outpatient and inpatient departments of the hospital during the same period as the control group.Diagnose the two groups of patients with natural night polysomnography.Observed the time in bed(TIB),sleep time(SPT),total sleep time(TST),wake time after falling asleep(WASO),and number of awakenings.(AT),wake time(ATA),sleep latency(SL),sleep efficiency(SE),N1,N2,N3,REM sleep period structure percentage,REM sleep time(RT),REM latency(RL),REM Number of cycles(NRP)situation.Results The SL and SPT of the experimental group were significantly longer than the control group,AT was significantly less than the control group,SE was significantly lower than the control group,ATA,TIB,WASO,TST were significantly shorter than the control group,the difference was statistically significant(P<0.05);the experimental group N2(52.44±0.19)%was significantly lower than the control group(57.77±1.05)%,N1(22.63±0.23)%,N3(18.57±0.28)%was significantly greater than the control group(22.13±0.19)%,(9.26±0.17)%,the structure percentage of REM sleep period(12.13±0.09)%was significantly smaller than the control group(12.52±0.03)%,the difference was statistically significant(t=31.592,10.600,179.755,26.000,P<0.05);RT(57.71±0.12)min and RL(148.53±1.26)min of the experimental group were significantly shorter than those of the control group(58.13±0.45)min and(166.79±1.49)min,and NRP(3.91±0.01)times was significantly higher than In the control group(3.26±0.07)times,the difference was statistically significant(t=5.704,59.183,58.138,P<0.05).Conclusion Natural night polysomnography has high clinical value for the diagnosis of patients with first-episode depression,and can effectively clarify the patients'TIB,SPT,TST,WASO,AT,ATA,SL,SE,N1,N2,N3,structure percentage in REM sleep period,RT,RL,NRP.
