腹腔镜胰体尾切除术10例临床分析

Clinical Analysis of 10 Cases of Laparoscopic Pancreatectomy

目的观察和分析腹腔镜胰体尾切除术操作方法以及治疗效果。方法选取31例自2018年5月—2019年10月进行手术治疗在该院接受胰体尾切除术患者,将之分为对比组(n=21)与研究组(n=10),对比组应用开腹胰体尾切除术,研究组应用腹腔镜胰体尾切除术,比较两组手术相关指标以及术后并发症发生情况。结果研究组手术时间(286.23±49.38)min较对比组(230.15±52.12)min长,差异有统计学意义(t=7.332,P<0.05);研究组其他各项手术相关指标均优于对比组,术中失血量[(351.22±78.35)mL vs(185.23±73.45)mL]更少,术后进食时间[(4.23±1.35)d vs(3.01±1.30)d],术后排气时间[(3.51±1.33)d vs(2.29±1.28)d]及总住院时间[(15.26±2.13)d vs(11.23±2.08)d]均明显更短,差异有统计学意义(t=21.038、3.015、4.282、6.348,P<0.05)。两组并发症发生率经对比差异无统计学意义(P>0.05)。结论腹腔镜胰体尾切除术治疗效果优于开腹胰体尾切除术,临床应用价值较高。能够使患者从中获益,可缩短患者术后恢复时间,具有一定的应用价值。

Objective To observe and analyze the operation method and treatment effect of laparoscopic pancreatectomy.Methods A total of 31 patients who underwent surgical treatment and undergoing pancreatic body and tail resection in the hospital from May 2018 to October 2019 were selected and divided into a control group(n=21)and a study group(n=10).The control group was treated with open pancreatic body and tail resection,and the study group was treated with laparoscopic pancreatic body and tail resection.The two groups were compared with the related indexes and postoperative complications.Results The operation time of the study group was(286.23±49.38)min longer than that of the control group(230.15±52.12)min,and the difference was statistically significant(t=7.332,P<0.05).The other operation-related indicators of the study group were better than those of the control group.Intraoperative blood loss[(351.22±78.35)mL vs(185.23±73.45)mL]was less,and postoperative eating time[(4.23±1.35)d vs(3.01±1.30)d],The postoperative exhaust time[(3.51±1.33)d vs(2.29±1.28)d]and the total hospital stay[(15.26±2.13)d vs(11.23±2.08)d],were significantly shorter,and the difference were statistically significant(t=21.038,3.015,4.282,6.348,P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The therapeutic effect of laparoscopic pancreatic body and tail resection is better than that of open pancreatic body and tail resection,and its clinical application value is higher.It can make patients benefit from it,can shorten the recovery time of patients after operation,and has certain application value.
