超声影像学在鉴别非哺乳期乳腺炎与乳腺癌中的应用价值分析 被引量:3

摘要 目的探讨多普勒彩超检查手段对非哺乳期乳腺炎和乳腺癌的鉴别诊断价值及两类乳腺疾病的超声影像学特征。方法选取2016年1月—2020年1月于该院就诊并经手术病理确诊的非哺乳期乳腺炎患者28例作为BI组(乳腺炎组),选取同期确诊的乳腺癌患者20例作为BC组(乳腺癌),比较两组诊断准确性和超声影像学表现特点。结果诊断符合率:BI组为85.7%(24/28),BC组为90.0%(18/20),两组符合率均超过80%,且比较差异无统计学意义(χ^(2)=0.000,P=1.000);影像学表现分布:BC组中边界欠清(80.0%)、形态不规则(70.0%)、钙化灶(75.0%)、腋窝淋巴结改变(95.0%)、血流信号分级≥II级(90.0%)患者占比显著高于BI组,差异有统计学意义(χ^(2)=6.622、5.486、7.215、9.692、11.063,P<0.05)。结论彩色多普勒超声技术对于非哺乳期乳腺炎和乳腺癌的诊断准确率均较高,且良、恶性乳腺疾病在超声影像学上具有显著差异,有助于进行临床鉴别。 Objective To investigate the value of Doppler color Doppler ultrasound in the differential diagnosis of non-lactating mastitis and breast cancer and the ultrasound imaging characteristics of the two types of breast diseases.Methods Twenty-eight non-lactating mastitis patients who were diagnosed by surgery and pathology in the hospital from January 2016 to January 2020 were selected as the BI group(mastitis group),and 20 breast cancer patients diagnosed during the same period were selected as BC group(breast cancer),compare the diagnostic accuracy and ultrasound imaging characteristics of the two groups.Results The diagnosis coincidence rate:85.7%(24/28)in the BI group and 90.0%(18/20)in the BC group.The coincidence rates of the two groups both exceeded 80%,and the difference was not statistically significant(χ^(2)=0.000,P=1.000);Distribution of imaging findings:in the BC group,the boundary was unclear(80.0%),and the shape was irregular(70.0%),calcification(75.0%),axillary lymph node changes(95.0%),the proportion of blood flow signal grade≥grade II(90.0%)was significantly higher than that in the BI group,and the difference was statistically significant(χ^(2)=6.622,5.486,7.215,9.692,11.063,P<0.05).Conclusion Color Doppler ultrasound technology has high diagnostic accuracy for non-lactating mastitis and breast cancer,and benign and malignant breast diseases have significant differences in ultrasound imaging,which is helpful for clinical differentiation.
