摘要
目的探究高压氧联合去骨瓣减压术在重型颅脑外伤中的应用。方法于2017年1月—2020年1月在合肥市第二人民医院神经外科进行治疗的200例重型颅脑外伤患者为研究对象,按照入院顺序分组原则,各100例,其中100例进行高压氧联合去骨瓣减压术治疗(观察组)、100例进行去骨瓣减压术治疗(对照组)。比较各组指标。结果观察组患者治疗后神经功能缺损评分(21.03±1.02)分、GCS评分(12.71±3.21)分与对照组相比,差异有统计学意义(t=32.778、5.519,P<0.05);观察组重型颅脑外伤患者治疗总有效率94.00%高于对照组,差异有统计学意义(χ^(2)=29.339,P<0.05);观察组重型颅脑外伤患者并发症发生率(2.00%)低于对照组,差异有统计学意义(χ^(2)=4.714,P<0.05)。结论高压氧联合去骨瓣减压术在重型颅脑外伤中具有较高应用价值,能够改善患者神经功能缺损情况,提高治疗效果。
Objective To explore the application of hyperbaric oxygen combined with decompressive craniectomy in severe traumatic brain injury.Methods Selected 200 patients with severe traumatic brain injury treated in the department of neurosurgery of Hefei Second People's Hospital from January 2017 to January 2020.According to the principle of grouping in the order of admission,100 cases in each group.Among them,100 cases were treated with hyperbaric oxygen combined with decompressive craniectomy(observation group),and 100 cases were treated with decompressive craniectomy(control group).The indexes of each group were compared.Results The scores of neurological impairment(21.03±1.02)points and GCS scores(12.71±3.21)points of the observation group after treatment were different from those of the control group,and the difference was statistically significant(t=32.778,5.519,P<0.05);The total effective rate of treatment in patients with severe head trauma in the observation group was 94.00%higher than that in the control group,and the difference was statistically significant(χ^(2)=29.339,P<0.05);the complication rate of patients in the observation group(2.00%)was lower than that in the control group,and the difference was statistically significant(χ^(2)=4.714,P<0.05).Conclusion Hyperbaric oxygen combined with decompressive craniectomy has high application value in severe craniocerebral trauma.It can improve patients'neurological deficits and improve the treatment effect.
