黄芪建中汤加减联合针刺治疗消化性溃疡

Treatment of Peptic Ulcer With Modified Huangqi Jianzhong Decoction Combined With Acupuncture

目的探究黄芪建中汤加减方联合针刺治疗消化性溃疡(脾胃虚寒型)的疗效。方法选取2016年6月-2019年6月本院门诊收治的消化性溃疡(脾胃虚寒型)患者96例作为研究对象,年龄在26~68周岁。根据门诊单双号分为治疗治疗组和对照组,每组48例。对照给予常规口服埃索美拉唑镁肠溶片、阿莫西林胶囊、克拉霉素缓释片、胶体果胶铋胶囊等,治疗组则给予黄芪建中汤加减联合针刺治疗,疗程6周。观察两组患者治疗有效率及不良反应,并采用专业量表调查患者治疗前后中医症候积分、简式McGill疼痛问卷结果。结果两组疗效对比显示,治疗组总有效率明显高于对照组,差异对比有统计学意义(P <0.05);两组患者在中医症候积分、简式McGill疼痛问卷各项积分方面均有显著降低,提示两组均有明显治疗效果。治疗组简式McGill疼痛问卷表中VAS、PPI及PRI(感觉项总分、情感项总分)、差异有统计学意义(P <0.05)。两组不良反应发生情况差异无统计学意义(P> 0.05)。结论黄芪建中汤联合针刺治疗可有效缓解消化性溃疡(脾胃虚寒型)患者中医症候及降低简式McGill疼痛问卷各项积分,疗效可靠,值得临床应用。

Objective To explore the curative effect of Huangqi Jianzhong decoction combined with acupuncture in the treatment of peptic ulcer(spleen stomach deficiency cold type).Methods 96 patients with peptic ulcer(spleen stomach deficiency cold type)in our hospital from June 2016 to June 2019 were selected as the research objects,aged from 26 to 68 years old.They were divided into treatment group and control group,48 cases in each group.The control group was given conventional oral esomeprazole magnesium enteric coated tablets,amoxicillin capsules,clarithromycin sustained-release tablets,colloidal bismuth pectin capsules,while the treatment group was given Huangqi Jianzhong decoction combined with acupuncture and moxibustion for 6 weeks.The effective rate and adverse reactions of the two groups were observed,and the TCM symptom score and short form McGill pain questionnaire before and after treatment were investigated by professional scale.Results The total effective rate of the treatment group was significantly higher than that of the control group,and the difference was significant(P<0.05);the TCM symptom score and the scores of the simplified McGill pain questionnaire of the two groups were significantly reduced,indicating that the two groups had obvious therapeutic effect.There were significant differences in VAS,PPI and PRI(total score of sensory items and total score of emotional items)in the treatment group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Huangqi Jianzhong decoction combined with acupuncture can effectively relieve the TCM symptoms of patients with peptic ulcer(spleen and stomach deficiency cold type)and reduce the scores of McGill pain questionnaire,which has reliable curative effect and is suitable for clinical application.
