缬沙坦与胺碘酮联合治疗慢性心功能不全合并阵发性房颤对其心功能的影响

The Effect of Valsartan and Amiodarone in the Treatment of Chronic Cardiac Insufficiency with Paroxysmal Atrial Fibrillation on Cardiac Function

目的探究缬沙坦与胺碘酮联合治疗慢性心功能不全合并阵发性房颤对其心功能的影响。方法方便选择2017年12月-2019年9月期间在该院进行治疗的慢性心功能不全合并阵发性房颤患者共96例,采取抽签法进行随机分组,对照组和观察组各48例。对照组开展胺碘酮治疗,观察组开展缬沙坦联合胺碘酮治疗,比较两组患者心功能分级、窦性心律维持率、左心室功能参数及不良反应发生率。结果观察组患者窦性心律维持率93.75%高于对照组79.17%,差异有统计学意义(χ^(2)=4.360,P<0.05);观察组左心室LVEDD(52.48±2.56)mm、LVESD(30.24±1.68)mm低于对照组(60.37±2.43)mm、(36.85±2.12)mm,左心室LVEF(48.73±2.31)%高于对照组(41.85±2.46)%,差异有统计学意义(t=15.487、16.930、14.125,P<0.05);观察组患者不良反应发生率8.33%低于对照组14.58%,差异无统计学意义(P>0.05)。结论慢性心功能不全合并阵发性房颤患者开展缬沙坦联合胺碘酮治疗可以有效改善其心功能不全症状,提升窦性心律维持率,且具有较高的安全性,因此可将该种治疗方案用于临床率大力推广。

Objective To explore the effect of valsartan and amiodarone in the treatment of chronic cardiac insufficiency combined with paroxysmal atrial fibrillation on cardiac function.Methods A total of 96 patients with chronic cardiac insufficiency and paroxysmal atrial fibrillation who were treated in the hospital from December 2017 to September 2019 were conveniently selected and randomized by lottery.The control group and the observation group each had 48 cases.The control group was treated with amiodarone,and the observation group was treated with valsartan combined with amiodarone.The cardiac function classification,sinus rhythm maintenance rate,left ventricular function parameters and incidence of adverse reactions were compared between the two groups.Results The maintenance rate of sinus rhythm in the observation group was 93.75%higher than that in the control group 79.17%,and the difference was statistically significant(χ^(2)=4.360,P<0.05);the observation group left ventricle LVEDD(52.48±2.56)mm,LVESD(30.24±1.68)mm lower than the control group(60.37±2.43)mm,(36.85±2.12)mm,left ventricular LVEF(48.73±2.31)%higher than the control group(41.85±2.46)%,the difference was statistically significant(t=15.487,16.930,14.125,P<0.05);the incidence of adverse reactions in the observation group was 8.33%lower than that in the control group 14.58%,but the difference was not statistically significant(P>0.05).Conclusion Valsartan combined with amiodarone therapy in patients with chronic cardiac insufficiency and paroxysmal atrial fibrillation can effectively improve the symptoms of cardiac insufficiency,increase the maintenance rate of sinus rhythm,and has a high safety.This treatment plan is used for clinical promotion.
