四肢创伤骨折后骨不连采取带锁髓内钉固定治疗的效果分析

Analysis of the Effect of Fixation with Interlocking Intramedullary Nail for Nonunion after Limb Trauma Fracture

目的分析四肢创伤骨折后骨不连采取带锁髓内钉固定治疗的效果。方法研究对该院2017年4月—2020年2月骨外科的52例四肢创伤骨折后骨不连患者进行了观察,应用信封法将患者分成两组,对照组(n=26)应用动力加压钢板治疗,观察组(n=26)则采用带锁髓内钉固定治疗,对比两组骨折恢复效果、愈合时间、引流量、出血量和并发症。结果观察组骨折恢复优良率为96.15%(25/26),对照组优良率是76.92%(20/26),组间差异有统计学意义(χ^(2)=4.127,P<0.05)。观察组骨折愈合时间(24.12±1.47)周,术后引流量(121.56±15.98)mL,术中出血量(204.65±82.53)mL,数据较之对照组差异有统计学意义(t=10.528、4.363、3.508,P<0.05);观察组术后并发症数据是3.85%(1/26),对照组是26.92%(7/26),组间差异有统计学意义(χ^(2)=5.442,P<0.05)。结论四肢创伤骨折后骨不连采取带锁髓内钉固定治疗骨折愈合效果较好,且恢复较快,损伤小,术后并发症少,值得推荐。

Objective To analyze the effect of interlocking intramedullary nail fixation for nonunion after limb trauma and fracture.Methods The study observed 52 patients with nonunion after limb trauma and fracture in the hospital.The patients were enrolled between April 2017 and February 2020.The patients were divided into two groups by the envelope method,the control group(n=26)with dynamic compression plate treatment,and the observation group(n=26)was treated with locking intramedullary nails.The fracture recovery effect,healing time,drainage,bleeding volume and complications of the two groups were compared.Results The excellent and good rate of fracture recovery in the observation group was 96.15%(25/26),and the excellent and good rate in the control group was 76.92%(20/26).The difference between the groups was statistically significant(χ^(2)=4.127,P<0.05).The fracture healing time of the observation group was(24.12±1.47)weeks,the postoperative drainage volume was(121.56±15.98)mL,and the intraoperative blood loss was(204.65±82.53)mL.Compared with the control group,the data was statistically significanty different(t=10.528,4.363,3.508,P<0.05);the postoperative complications data of the observation group was 3.85%(1/26),and the control group was 26.92%(7/26).The difference between the groups was statistically significant(χ^(2)=5.442,P<0.05).Conclusion The treatment of nonunion with interlocking intramedullary nails after traumatic fractures of limbs has a better healing effect,faster recovery,less damage,and fewer postoperative complications.It is worth recommending.
