摘要 目的血清甲胎球蛋白、a-L-岩藻苷酶、糖类抗原199联合检测在原发性肝癌(PHC)诊断中的临床应用价值。方法该院2019年6—12月住院原发性肝癌患者(PHC)63例为原发性肝癌组,随机选取该院同期经实验室检查和临床确诊的慢性病毒性肝炎56例为肝炎组,该院同期健康体检者56名为对照组,使用罗氏E411全自动化学发光分析仪分别测3组AFP、CA199的浓度,使用日立7180自动生化分析仪分别测3组测定AFU浓度。结果PHC、慢性肝炎组及对照组AFP的浓度分别为(376.6±86.7)ng/mL、(7.3±4.08)ng/mL、(3.96±2.17)ng/mL;CA199的浓度分别为(199.3±48.9)U/mL、(31.6±10.5)U/mL、(19.5±8.7)U/mL;AFU的浓度分别为(99.8±29.8)μ/L、(35.8±10.5)μ/L、(25.8±9.9)μ/L。PHC组与慢性肝炎组、对照组比较差异有统计学意义(P<0.05),联合检测性的敏感性(95.2%)、准确度(80.7%)同单项检测比较,差异有统计学意义(P<0.05)。结论联合测定AFP、AFU和CA199在PHC诊断中能提高PHC的阳性检出率和诊断准确性。 Objective The clinical value of combined detection of serum alpha-fetoglobulin,a-L-fucosidase and carbohydrate antigen 199 in the diagnosis of primary liver cancer(PHC).Methods From June to December 2019 in the hospital,63 patients with primary liver cancer(PHC)were in the primary liver cancer group.56 patients with chronic viral hepatitis diagnosed by laboratory examination and clinical diagnosis in the hospital during the same period were randomly selected as hepatitis group,56 healthy persons in the hospital during the same period served as the control group.Roche E411 automatic chemiluminescence analyzer was used to measure the concentrations of AFP and CA199 in the three groups,and the Hitachi 7180 automatic biochemical analyzer was used to measure the AFU concentrations in the three groups.Results The concentrations of AFP in PHC,chronic hepatitis group and control group were(376.6±86.7)ng/mL,(7.3±4.08)ng/mL,(3.96±2.17)ng/mL,respectively;the concentration of CA199 was(199.3±48.9)U/mL,(31.6±10.5)U/mL,(19.5±8.7)U/mL;the concentration of AFU was(99.8±29.8)μ/L,(35.8±10.5)μ/L,(25.8±9.9)μ/L.The difference between the PHC group and the chronic hepatitis group and the control group was statistically significant(P<0.05).The sensitivity(95.2%)and accuracy(80.7%)of the combined detection were compared with the single test,and the difference was statistically significant(P<0.05).Conclusion Combined determination of AFP,AFU and CA199 can improve the positive detection rate and diagnostic accuracy of PHC in the diagnosis of PHC.
