活血散瘀定痛汤联合氨基葡萄糖治疗创伤性膝骨性关节炎临床观察

Clinical Observation on Huoxue Sanyu Dingtong Decoction Combined with Glucosamine in the Treatment of Traumatic Knee Osteoarthritis

目的探讨活血散瘀定痛汤联合氨基葡萄糖治疗创伤性膝骨性关节炎患者的临床效果。方法选取2018年9月—2020年9月在葫芦岛市中心医院进行治疗的创伤性膝骨性关节炎患者68例为研究对象,采用随机数字表法分为观察组和对照组,各34例。对照组给予氨基葡萄糖治疗,观察组在对照组治疗基础上联合活血散瘀定痛汤对患者进行治疗,比较2组疗效,分析对患者膝关节活动度的影响。结果观察组治疗总有效率97.06%(33/34)高于对照组的82.35%(28/34)(P<0.05);治疗后,2组关节红肿、关节疼痛、活动受限评分均降低,且观察组以上评分均明显低于对照组(P<0.05);治疗后,2组膝关节活动度、Lysholm评分改善情况均显著优于对照组(P<0.001);治疗后,2组肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平均降低,且观察组以上指标均明显低于对照组(P<0.05)。结论活血散瘀定痛汤联合氨基葡萄糖对创伤性膝骨性关节炎患者疗效较佳,可显著改善患者中医证候积分、膝关节活动度及膝关节功能,降低机体炎性反应,值得在临床上推广应用。

Objective To explore the clinical effect of Huoxue Sanyu Dingtong decoction combined with glucosamine on traumatic knee osteoarthritis(KOA).Methods A total of 68 patients with traumatic KOA who were treated in the hospital from September 2018 to September 2020 were enrolled as the research objects.They were divided into observation group and control group by random number table method,34 cases in each group.The control group was treated with glucosamine,while observation group was treated with Huoxue Sanyu Dingtong decoction combined with glucosamine.The curative effect was compared between two groups.The effects on knee ROM were analyzed.Results The total effective rate of the observation group was 97.06%(33/34)higher than that of the control group(82.35%,28/34)(P<0.05).After treatment,the scores of joint swelling,joint pain and motion limitation in the two groups were decreased,and the scores in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the improvement of knee motion and Lysholm score in the two groups were significantly better than that in the control group(P<0.001).After treatment,the levels of tumor necrosis factor-α(TNF-α)and hypersensitive C-reactive protein(hs-CRP)in the two groups were decreased,and the above indexes in observation group were significantly lower than those in control group(P<0.05).Conclusion Huoxue Sanyu Dingtong decoction combined with glucosamine has a better effect on patients with traumatic KOA,which can significantly improve the traditional Chinese medicine syndrome score,knee joint activity and knee joint function,reduce the inflammatory response of the body,and is worthy of clinical promotion and application.
