碳酸司维拉姆和醋酸钙片对维持性血液透析患者钙磷代谢紊乱和肾功能的影响

Effects of Sviram Carbonate and Calcium Acetate Tablets on Calcium and Phosphorus Metabolism Disorder and Renal Function in Maintenance Hemodialysis Patients

目的:研究碳酸司维拉姆和醋酸钙片对维持性血液透析患者钙磷代谢紊乱和肾功能的影响。方法:选取2017年6月~2019年3月在某院肾内科做维持性血液透析的100例患者作为研究对象,使用随机数字表法将患者分为试验组和对照组各50例,给予对照组患者醋酸钙片治疗,给予试验组患者碳酸思维拉姆治疗,治疗4周后比较两组患治疗后血清钙、血清磷、甲状旁腺激素、钙磷乘积水平及肾功能指标变化,比较两组患者治疗后不良并发症的发生率。结果:经治疗后试验组患者血清钙、血清磷、甲状旁腺激素、钙磷乘积显著低于对照组(P<0.05);治疗后两组患者肌酐和尿素氮水平与治疗前比较均降低,组间治疗后比较,差异无统计学意义(P>0.05);试验组患者治疗后腹泻、便秘、恶心、呕吐并发症发生率低于对照组,但差异无统计学意义(P>0.05)。结论:碳酸司维拉姆治疗维持性血液透析患者能有效降改善患者钙磷代谢紊乱,改善患者肾功能。

Objective:To study the effects of sviram carbonate and calcium acetate tablets on calcium and phosphorus metabolism disorder and renal function in maintenance hemodialysis patients.Methods:100 patients with maintenance hemodialysis in a hospital from June 2017 to March 2019 were selected as the research objects and divided into the experimental group(50 cases)and the control group(50 cases)by using the random number table method.The patients in the control group were treated with calcium acetate tablets,and the patients in the experimental group were treated with sviram carbonate for 4 weeks.The changes of serum calcium,serum phosphorus,parathyroid hormone,calcium phosphorus product and renal function indexes were compared between the two groups,and the incidence of adverse complications was compared.Results:After treatment,the serum calcium,serum phosphorus,parathyroid hormone and calcium phosphorus product in the experimental group were significantly lower than those in the control group(P<0.05).After treatment,the creatinine and urea nitrogen levels in the two groups were lower than those before treatment.There was no significant difference between the two groups after treatment(P>0.05).The diarrhea,constipation,nausea and vomiting in the experimental group were lower than those in the control group,but there was no significant difference(P>0.05).Conclusion:The calcium and phosphorus metabolism disorder and renal function of the patients with maintenance hemodialysis can be effectively reduced and improved by the treatment of sirolimus carbonate.
