硝矾熏洗剂对混合痔外剥内扎术患者肉芽生长情况及并发症的影响

Effect of nitre and alum fumigation lotion on granulation growth and complications in patients with hemorrhoids milligant-morgant hemorrhoidectomy

目的观察硝矾熏洗剂对混合痔外剥内扎术患者肉芽生长情况及并发症的影响。方法选取2018年3月至2019年1月在三门峡市第三人民医院普外科行混合痔外剥内扎术治疗的患者92例为研究对象,按随机双盲法将其分为两组,各46例。两组患者均接受混合痔外剥内扎术治疗,对照组术后给予常规的凡士林纱条塞入创面进行治疗,在此基础上,观察组给予硝矾熏洗剂治疗。观察两组术后创面疼痛程度、创面水肿、肉芽生长、创面愈合及并发症情况。结果对照组术后3 d、7 d VAS评分(5.35±1.25)分、(2.22±0.46)分均高于观察组(4.85±1.02)分、(1.86±0.25)分,差异有统计学意义(P<0.05);对照组创面水肿评分(1.85±0.24)分、肉芽生长评分(0.48±0.25)分均高于观察组(1.52±0.15)分、(0.24±0.14)分,创面愈合时间(17.98±4.35)d长于观察组(12.25±3.46)d,差异有统计学意义(P<0.05);较对照组(17.39%)相比,观察组术后并发症发生率(4.35%)低,差异有统计学意义(P<0.05)。结论硝矾熏洗剂治疗可有效缓解混合痔外剥内扎术患者创面疼痛程度,降低术后并发症发生率,利于促进创面肉芽组织生长,缩短创面愈合时间。

Objective To observe the effect of nitre and alum fumigation lotion on granulation growth and complications in patients with hemorrhoids milligant-morgant hemorrhoidectomy.Methods Totally 92 patients with hemorrhoids milligant-morgant hemorrhoidectomy in Department of General Surgery,Sanmenxia Third People’s Hospital from March 2018 to January 2019 were divided into two groups by randomized double blind method,with 46 cases in each group.Patients in 2 groups received hemorrhoids milligant-morgant hemorrhoidectomy for treatment.After surgery,the control group was treated with vaseline gauze packing,and observation group was treated with nitre and alum fumigation lotion.The postoperative wound pain degree,wound edema,granulation growth,wound healing and complications in two groups were observed.Results The VAS scores of the control group 3 days and 7 days after operation were(5.35±1.25)points and(2.22±0.46)points,higher than(4.85±1.02)points and(1.86±0.25)points of the observation group,with a statistical significant difference(P<0.05);the wound edema score(1.85±0.24)points and granulation growth score(0.48±0.25)points in the control group were higher than(1.52±0.15)points and(0.24±0.14)points in the observation group,and the wound healing time was(17.98±4.35)d longer than(12.25±3.46)d of the observation group,with a statistical significant difference(P<0.05);compared with the control group(17.39%),the incidence of postoperative complications in the observation group(4.35%)was lower,with a statistical significant difference(P<0.05).Conclusion Nitre and alum fumigation lotion in treatment of patients with hemorrhoids milligant-morgant hemorrhoidectomy can relieve wound pain,decrease the incidence of postoperative complications,promote wound granulation growth,and shorten wound healing time.
