某院139例难治性支原体肺炎患儿院内感染的相关因素分析及其对策

Analysis of Related Factors of Nosocomial Infection in Children with Refractory Mycoplasma Pneumonia and Its Countermeasures

目的:探究难治性支原体肺炎(RMPP)患儿院内感染的相关因素及其治疗对策。方法:选取2018年4月—2020年4月间治疗的RMPP患儿139例临床资料,根据院内感染发生情况,分为院内感染组58例,非感染组81例;比较与分析两组患儿发生RMPP的单因素和多因素与发生医院感染的相关性,如患儿性别、年龄和住院时间、特异体质、居住环境、侵入性操作和发病季节的发生情况,以及白细胞计数、C-反应蛋白、补体C3测得值的差异。结果:感染组患儿发生RMPP与年龄、患儿住院时间、侵入性操作、CRP水平以及补体C3水平具有相关性,其与非感染组比较其差异有统计学意义(P<0.05);而患儿年龄大于2岁、住院时间大于14 d、有侵入性操作、CRP水平高及补体C3水平低均是院内感染的独立危险因素(P<0.05)。结论:RMPP患儿发生院内感染的影响因素较多,临床应采取相应的干预措施,以减少院内感染的发生,确保患儿早日康复。

Objective:To explore the related factors of nosocomial infection in children with refractory mycoplasma pneumonia(RMP)and its countermeasures.Methods:The clinical data of 139 children with RMP treated from April 2018 to April 2020 were selected.Based on the incidence of nosocomial infection,the patients were divided into the nosocomial infection group(58 cases)and the non-infection group(81 cases).To compare and analyze the correlation between single and multiple factors of RMP and nosocomial infection in children in the two groups,such as gender,age and length of stay,specific constitution,living environment,invasive operation and incidence season,as well as the differences in white blood cell count,C-reactive protein and complement C3 values were compared and analyzed.Results:The incidence of RMPP in the infected group was correlated with age,length of hospital stay,invasive operation,CRP level and complement C3 level,and the difference was statistically significant compared with the non-infected group(P<0.05).However,children aged more than 2 years,length of hospital stay greater than 14 days,invasive operation,high CRP level and low complement C3 level were independent risk factors for nosocomial infection(P<0.05).Conclusion:There were many influencing factors for nosocomial infection in children with RMP,and corresponding intervention measures had be taken clinically to reduce the occurrence of nosocomial infection and ensure the early recovery of children.
