矢量定位法细孔钻颅治疗基底节区脑出血的临床应用 被引量:2

摘要 目的探讨矢量定位法细孔钻颅治疗基底节脑出血的效果及应用价值。方法回顾性分析40例高血压性基底节脑出血患者的临床资料;其中20例患者应用矢量定位法经额细孔钻颅穿刺引流血肿(矢量组),20例患者采用传统定位经额细孔钻颅穿刺引流血肿(传统组)。对两组患者的首次穿刺成功率、术中调管次数、穿刺道出血率以及术后血肿清除率指标进行分析比较。结果矢量组患者的穿刺成功率明显高于传统组,术中调管次数明显少于传统组,差异均有统计学意义(均P<0.05);两组患者的穿刺道出血率和血肿清除率比较,差异无统计学意义(均P>0.05)。结论矢量定位法治疗基底节脑出血具有微创、准确、安全等特点,值得在临床实践中推广使用。 Objective To explore the effect and application of vector positioning method with fine-hole cranial drilling in the treatment of basal ganglia cerebral hemorrhage.Methods The clinical data of 40 patients with hypertensive basal ganglia hemorrhage were analyzed retrospectively.Of 40 cases,20 cases treated with vector positioning through the frontal hole drilling puncture and drainage were devided into the vector group.20 cases treated with traditional positioning through the forehead drilling puncture and drainage were the traditional group.The success rate of the first puncture,intraoperative frequency of tube adjustment,puncture tract bleeding rate and the clearance rate of postoperative hematoma were compared between the two groups.Results The success rate of puncture in the vector group was significantly better than that in the traditional group,the frequency of tube adjustment was significantly less than that in the traditional group,the two differences were statistically significant(P<0.05).But there was no statistically significant in the puncture bleeding rate and hematoma clearance rate between the two groups(P>0.05).Conclusion The vector positioning method for the treatment of basal ganglia cerebral hemorrhage is minimally invasive,accurate and safe,and which is worth popularizing in clinical practice.
