针灸治疗膝骨关节炎作用机制研究

Research on the Mechanism of Acupuncture Treatment of Knee Osteoarthritis

膝骨关节炎(knee osteoarthritis,KOA)是一种以膝周疼痛、关节功能下降为临床特征的退行性病变,其发病机理复杂,具有高发病率、高致残率的特点,严重影响患者生活质量。针灸作为治疗膝骨关节炎的一种特色疗法,操作简便,疗效确切。大量研究证实,针灸疗法干预膝骨关节炎可通过降低炎性因子表达,减少局部炎症刺激;抑制软骨细胞凋亡,促进软骨细胞增殖;平衡软骨细胞外基质的合成与降解;增强膝周肌力,促使应力平衡;改善局部微循环,降低骨内压;调节疼痛介质水平,发挥镇痛功效等作用,从而能够明显减轻疼痛症状,改善膝关节功能,提高患者生活质量。笔者阅览近几年关于针灸治疗膝骨关节炎的文献,对其作用机制进行分析并总结,做一综述,以期为以后更深入的研究提供一定参考。

Knee osteoarthritis(KOA)is a degenerative disease characterized by pain around the knee and decreased joint function.Its pathogenesis is complex,with high morbidity and high disability rate,and it is seriously affected patient quality of life.As a special treatment for knee osteoarthritis,acupuncture therapyare is easy to operate and has definite curative effect.A large number of studies have confirmed that acupuncture intervention in knee osteoarthritis can reduce the expression of inflammatory factors and reduce local inflammatory stimulation;inhibit chondrocyte apoptosis and promote chondrocyte proliferation;balance the synthesis and degradation of chondrocyte extracellular matrix;enhance knee muscle strength,promote stress balance;improve local microcirculation,reduce intraosseous pressure;regulate the level of pain medium and exert analgesic effect,etc,which can significantly reduce pain symptoms,improve knee function,and improve the quality of life of patients.Author has read the literatures on acupuncture treatment of knee osteoarthritis in recent years,analyze and summarize its mechanism of action,and make a review in order to provide a certain reference for future in-depth research.
