慢性心力衰竭的阴阳辨证与NT-proBNP及射血分数的相关性研究

Study on the Correlation between Yin and Yang Syndrome Differentiation of Chronic Heart Failure,NT-proBNP and Ejection Fraction

目的研究慢性心力衰竭的阴阳辨证与NT-proBNP及射血分数的相关性。方法选择2018年7月—2019年7月就诊的慢性心力衰竭患者99例,对每例患者进行中医证候阴阳辨证分型,并检测患者同型半胱氨酸(Hcy)、超敏CRP(hs-CRP)、氨基末端脑钠肽前体(NT-proBNP)值、射血分数,记录结果并进行比较,分析慢性心力衰竭的阴阳辨证与同型半胱氨酸、超敏CRP、NTpro BNP(氨基末端脑钠肽前体)及射血分数的相关性。结果慢性心力衰竭主要证型包括心肺气虚型、气虚血瘀证、脾肾阳虚证,随着证型的不断转变,患者NT-proBNP不断升高,左心室射血分数不断降低;Ⅱ级、Ⅲ级、Ⅳ级之间同型半胱氨酸、超敏CRP、NTpro BNP、射血分数具有显著差异,数据对比差异具有统计学意义(P<0.05);不同中医证型患者同型半胱氨酸、超敏CRP、NTpro BNP、射血分数之间具有统计学差异,数据对比差异具有统计学意义(P<0.05)。结论慢性心力衰竭的阴阳辨证与同型半胱氨酸、超敏CRP、NT-proBNP及射血分数具有一定相关性,临床进行相关辨证时可进行参考。

Objective To study the correlation between yin and yang syndrome differentiation of chronic heart failure and NT-proBNP and ejection fraction.Method 99 patients with chronic heart failure who were treated in our hospital from July 2018 to July 2019 were selected,and each patient was classified into yin and yang dialectics of TCM syndromes,and the patients were tested for homocysteine(Hcy),Hypersensitivity CRP(hs-CRP),NT-proBNP value,ejection fraction,record the results and compare them,analyze the yin and yang syndrome of chronic heart failure and homocysteine,hypersensitivity CRP,NT-proBNP(amino terminal brain sodium Peptide precursor)and the correlation of ejection fraction.Results The main types of chronic heart failure include heart and lung qi deficiency,qi deficiency and blood stasis syndrome,and spleen and kidney yang deficiency.As the syndrome changes,the patient’s NT-proBNP(amino-terminal probrain natriuretic peptide)continues to increase,left The ventricular ejection fraction continued to decrease;homocysteine,hypersensitivity CRP,NT-proBNP,ejection fraction were significantly different between grade Ⅱ,grade Ⅲ,and gradeⅣ,and the data comparison was statistically significant(P<0.05).There are significant differences in homocysteine,hypersensitivity CRP,NT-proBNP,ejection fraction among patients with different TCM syndromes,and the data comparison was statistically significant(P<0.05).Conclusion The yin and yang syndrome differentiation of chronic heart failure has a certain correlation with homocysteine,hypersensitivity CRP,NT-proBNP and ejection fraction,which can be used as a reference for clinical diagnosis.
