美沙拉秦灌肠液联合双歧杆菌三联活菌肠溶胶囊治疗溃疡性结肠炎的临床效果及对患者炎症因子水平的影响

Clinical effect of mesalazine enema liquid combined with bifid triple viable capsules dissolving at intestines on ulcerative colitis and its influence on the levels of inflammatory factors in patients

目的观察美沙拉秦灌肠液联合双歧杆菌三联活菌肠溶胶囊治疗溃疡性结肠炎的临床效果及对患者炎症因子水平的影响。方法选取湖南中医药高等专科学校附属第一医院消化内科2018年3月至2019年12月治疗的100例溃疡性结肠炎患者作为研究对象,按照随机数字表法分为观察组和对照组,各50例。对照组采用美沙拉秦灌肠液睡前保留灌肠治疗,观察组在对照组基础上口服双歧杆菌三联活菌肠溶胶囊治疗,2组患者均治疗8周。分别比较2组临床有效率、生活质量评分、炎症因子[白细胞介素6(IL-6)、IL-8、IL-10、C反应蛋白(CRP)及肿瘤坏死因子α(TNF-α)]水平变化情况。比较2组并发症发生情况。结果观察组临床有效率高于对照组[92.0%(46/50)比70.0%(35/50)],差异有统计学意义(P<0.05)。治疗后2组生活质量评分均较治疗前升高,且观察组高于对照组(均P<0.05)。治疗后,2组IL-6、IL-8、CRP、TNF-α水平均较治疗前明显降低、且观察组低于对照组[(128±13)ng/L比(141±10)ng/L、(155±24)ng/L比(194±37)ng/L、(24±5)mg/L比(43±9)mg/L、(17±6)ng/L比(23±9)ng/L],IL-10水平较治疗前明显升高、且观察组高于对照组[(68±12)ng/L比(49±10)ng/L],差异均有统计学意义(均P<0.05)。观察组并发症发生率低于对照组[4.0%(2/50)比20.0%(10/50)],差异有统计学意义(P<0.05)。结论美沙拉秦灌肠液联合双歧杆菌三联活菌肠溶胶囊治疗溃疡性结肠炎能有效缓解临床症状,减少并发症发生率,提高患者生活质量,降低促炎因子水平,提高抑炎因子水平。

Objective To observe the clinical effect of mesalazine enema liquid combined with bifid triple viable capsules dissolving at intestines on ulcerative colitis and its influence on the levels of inflammatory factors in patients.Methods From March 2018 to December 2019,100 patients with ulcerative colitis treated in Department of Gastroenterology of the First Affiliated Hospital of Hunan Traditional Chinese Medical College were selected.They were randomly divided into observation group and control group,with 50 cases in each group.The control group was treated with mesalazine enema liquid retention enema before sleep,and the observation group was treated with bifid triple viable capsules dissolving at intestines orally on the basis of the control group.Both groups were treated for 8 weeks.The clinical effective rate,quality of life score,levels of inflammatory factors[interleukin-6(IL-6),IL-8,IL-10,C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)]and complications were compared between the two groups.Results The clinical effective rate of the observation group was higher than that of the control group[92.0%(46/50)vs 70.0%(35/50)](P<0.05).After treatment,the quality of life scores of the two groups were higher than those before treatment,and that of the observation group was higher than that of the control group(all P<0.05).After treatment,the levels of IL-6,IL-8,CRP and TNF-αof the two groups were significantly lower than those before treatment,and those of the observation group were lower than those of the control group[(128±13)ng/L vs(141±10)ng/L,(155±24)ng/L vs(194±37)ng/L,(24±5)mg/L vs(43±9)mg/L,(17±6)ng/L vs(23±9)ng/L];the levels of IL-10 of both groups were significantly higher than those before treatment,and that of the observation group was higher than that of the control group[(68±12)ng/L vs(49±10)ng/L](all P<0.05).The incidence of complications of the observation group was lower than that of the control group[4.0%(2/50)vs 20.0%(10/50)],and the difference was statistically significant(P<0.05).Conclusion Mesalazine enema liquid combined with bifid triple viable capsules dissolving at intestines in the treatment of ulcerative colitis can effectively alleviate clinical symptoms,reduce the incidence of complications,improve patients’quality of life,reduce the levels of pro-inflammatory factors,and increase the levels of anti-inflammatory factors.
