卵巢储备功能下降的不孕症女性抗苗勒管激素与体重指数的相关性研究 被引量:16

摘要 目的探究卵巢储备功能下降(DOR)不孕症患者抗苗勒管激素(AMH)水平与体重指数(BMI)的相关性。方法回顾性分析2016年6月至2019年6月于郑州大学第一附属医院生殖医学中心就诊的4010例DOR不孕症患者临床资料,根据年龄分为低龄组(≤35岁,n=1857)和高龄组(>35岁,n=2153),各组再根据BMI分为正常体重组(18.5 kg/m^(2)≤BMI<24 kg/m^(2))和超重及肥胖组(BMI≥24 kg/m^(2)),比较各组患者的血清AMH水平及其他潜在影响AMH的因素。结果多重线性回归分析显示:低龄组AMH水平与BMI呈负相关(β=-0.100,P<0.05);高龄组AMH水平与BMI无显著相关性(P>0.05)。此外,高龄组AMH水平与基础卵泡刺激素(bFSH)水平呈负相关(P<0.05),低龄组AMH与bFSH水平无显著相关性(P>0.05)。不同年龄组AMH水平均与窦卵泡数(AFC)呈正相关(P<0.05),与年龄无显著相关性(P>0.05)。结论≤35岁DOR不孕症女性BMI是血清AMH水平变化的独立影响因子,而>35岁DOR不孕症女性BMI与血清AMH水平变化无关。DOR不孕症患者的年龄不能独立预测血清AMH水平。 Objective:To explore the correlation between anti-Müllerian hormone(AMH)and body mass index(BMI)in infertile patients with diminished ovarian reserve(DOR).Methods:A retrospective analysis was conducted on the clinical data of 4010 infertile patients with DOR who were treated in the Reproductive Medicine Center of the First Affiliated Hospital of Zhengzhou University from June 2016 to June 2019.They were divided into the young group(≤35 years old,n=1857)and the old group(>35 years old,n=2153)according to age,and each group was further subdivided into normal weight group(18.5 kg/m^(2)≤BMI<24 kg/m^(2))and overweight and obese group(BMI≥24 kg/m^(2))according to BMI.The serum AMH level and other potential factors affecting AMH were compared among the groups.Results:The serum AMH level in the young group was negatively correlated with BMI(β=-0.100,P<0.05).However,there was no significant correlation between serum AMH level and BMI in the old group(P>0.05).In addition,there was a negative correlation between serum AMH level and bFSH level in the old group(P<0.05),but there was no significant correlation in the young group(P>0.05).The serum AMH level was positively correlated with the antral follicle count(AFC)in different age groups(P<0.05),but not significantly correlated with age(P>0.05).Conclusions:BMI is an independent influencing factor of serum AMH level among young infertile women with DOR(≤35 years),while BMI is not related to the change of serum AMH level among old infertile women with DOR(>35 years).Age of infertility women with DOR cannot independently predict serum AMH level.
