改良型强迫长期卧位法联合Gufoni法治疗向地性眼震型水平半规管良性阵发性位置性眩晕效果对照研究

A comparative study on the effect of modified FPP maneuvers combined with Gufoni maneuvers in the treatment of geotropic horizontal semicircular canal benign paroxysmal positional vertigo

目的:评估并探讨改良型强迫长期卧位法(FPP法)联合Gufoni法在治疗向地性眼震型水平半规管良性阵发性位置性眩晕中的效果以及在临床推广应用的可行性。方法:采用前瞻性、开放登记的研究方法纳入80例向地性眼震型水平半规管良性阵发性位置性眩晕患者,并随机分为两组,每组40例。第1天分别行单纯药物治疗及药物联合改良型FPP法复位治疗,第2天应用滚转试验(Roll-test)评估两种治疗方法的效果;未痊愈者再采用Gufoni法进行手法复位1次,复位结束30 min后采用Roll-test评估两组患者复位后即刻疗效。结果:单纯药物治疗与药物联合改良型FPP法复位治疗比较,单纯药物治疗治愈率15%,药物联合改良型FPP法治愈率55%,两组间治愈率差异有统计学意义(P<0.05);单纯药物治疗与药物联合改良型FPP法治疗两组间未痊愈者,再行Gufoni法复位治疗效果比较,两组间复位Gufoni治愈率分别是58.8%和94.4%,两组间差异有统计学意义(P<0.05)。结论:药物联合改良型FPP法复位治疗比单纯药物治疗有显著的治疗效果;经过改良型FPP法治疗后,再行Gufoni法复位治疗有更显著的临床治愈率。

Objective To evaluate and discuss the efficacy of modified forced prolonged position and Gufoni maneuvers for treatment of patients with geotropic type of benign paroxysmal positional vertigo of horizontal semicircular canal(HSC-BPPV).Method A randomized,controlled clinical trial was conducted,80 consecutive patients with geotropic HSC-BPPV were randomly assigned to Simple drug(n=40)and modified FPP(n=40)for treatment on the first day.The roll-test was used to evaluate the effect of the two treatments on next day,executhe unhealed patients was executed with Gufoni maneuvers once.The immediate therapeutic efficacy was compared within 30 min after maneuvers.Results Compared with Simple drugs and modified FPP combined with drugs,the cure rate of Simple drugs was 15%,while the cure rate of FPP combined with drugs was 55%The cure rate of the two groups were statistically significant(P<0.05).the unhealed patients between Simple drugs and modified FPP combined with drugs were executed with Gufoni maneuvers.the cure rate with Gufoni maneuvers between two groups was 58.8%and 94.4%,respectively,there was obvious statistical significance(P<0.05).Conclusion Modified FPP combined with drugs has a significant therapeutic effect than Simple drugs,After the modified FPP method,There is a more significant clinical cure rate with with Gufoni maneuvers.
