腕踝针结合体针治疗急性腰扭伤临床研究

Clinical Study of Wrist-Ankle Acupuncture Combined with Body Acupuncture in Treating Acute Lumbar Sprain

目的:观察腕踝针结合体针治疗急性腰扭伤的临床疗效,为腕踝针作为一种简单、易行、安全和有效的中医临床适宜技术推广提供部分临床依据。方法:纳入符合急性腰扭伤诊断标准的病例,随机分为结合组和体针组,各75例,疗程3 d。采用视觉模拟评分(VAS)及重复改良的Schober试验(MMS)为主要评价指标,结合中医病证诊断疗效标准及炎症因子白介素IL-10、白介素IL-6水平,观察腕踝针结合体针治疗急性腰扭伤的临床疗效。结果:结合组总有效率为89.33%(67/75),优于体针组的74.67%(56/75)(P<0.05)。结合组治疗第1天的VAS评分改善与体针组比较,差异有统计学意义(P<0.05)。结合组第3天的MMS评分改善与体针组比较,差异有统计学意义(P<0.05)。治疗后两组IL-6、IL-10水平明显降低(P<0.05),且结合组IL-6、IL-10含量明显低于体针组(P<0.05)。结论:腕踝针结合体针治疗急性腰扭伤的临床疗效确切,值得基层医疗单位推广应用。

Objective:To observe the clinical efficacy of wrist-ankle acupuncture combined with body acupuncture in the treatment of acute lumbar sprain,and to provide some clinical basis for the promotion of wrist-ankle acupuncture as a simple,easy,safe and effective clinical appropriate technology of traditional Chinese medicine.Methods:150 patients who met the diagnostic criteria of acute lumbar sprain were randomly divided into the combined group(n=75)and the body acupuncture group(n=75).After 3 days of the treatment,the scores of visual analogue scale(VAS)and repeated modified Schober test(MMS)were used as the main evaluation indicators,combined with the criteria of TCM syndrome diagnosis and curative effect,and the levels of IL-10 and IL-6 to observe the clinical efficacy of wrist-ankle acupuncture combined with body acupuncture in the treatment of acute lumbar sprain.Results:The total effective rate of the combined group was 89.33%(67/75),which was significantly higher than 74.67%(56/75)of the body acupuncture group(P<0.05).The improvement of VAS score on the first day of the treatment was more significant in the combined group compared to that in the body acupuncture group(P<0.05).The improvement of MMS score on the third day was more significant in the combined group than that of the body acupuncture group(P<0.05).The levels of IL-6 and IL-10 were significantly decreased after the treatment in the two groups(P<0.05);the decreases in the combined group were more significant(P<0.05).Conclusion:Wrist-ankle acupuncture combined with body acupuncture is effective in the treatment of acute lumbar sprain,which is worthy of popularization and application in primary medical units.
