贵州省药事管理质量控制指标(2020版)实施现状调查研究

Status of New Quality Control Indices(2020) of Pharmaceutical Administration in Guizhou Province: a survey of hospital pharmacies

目的:调查新版贵州省药事管理质量控制指标(2020)的实施现状。方法:贵州省药事管理质量控制中心采用问卷形式调查全省各医疗机构各项药事管理质量控制指标的执行情况。采用χ2检验、Fisher确切概率法、t检验或Mann-Whitney秩和检验进行单因素分析,探讨医疗机构级别及临床药师对这些指标的影响。结果:共调查全省66家县级以上医疗机构,我省药学专业技术人员(均数:3.82%)以及临床药师(中位数:0.42人/100床)的数量短缺。只有少数几家医院能够开展处方医嘱前置审核(n=8)以及静脉用药集中调配(n=5)工作,而能够统计报告住院患者药学监护率、用药错误报告率、住院患者中药静脉输液使用率、急诊患者糖皮质激素静脉输液使用率以及质子泵抑制剂注射剂静脉使用率这5项指标的医院不足50%。结论:贵州省各项药事管理工作的开展不能达到新指标体系要求,而药学技术人员和临床药师人力资源不足是制约这些工作开展最主要的原因。

OBJECTIVE To explore the status of new quality control indices of pharmaceutical administration in Guizhou Province.METHODS Based at Quality Control Center of Guizhou Pharmaceutical Administration, the survey was conducted by submitting the questionnaire to the directors of hospital pharmacy at 66 hospitals.Chi-square, Fisher’s exact, t or Mann-Whitney U test was utilized for univariate analysis for exploring the effect of institution rank or clinical pharmacy on various indices.RESULTS Pharmaceutical professionals(mean: 3.82%)and clinical pharmacists(median: 0.42 person per 100 beds)were insufficient in Guizhou.Only few hospitals could provide pre-prescription auditing(n=8)or pharmacy intravenous admixture service(n=5).Less than half of them were capable of reporting pharmaceutical care rate, medication error rate, rate of intravenous infusion of glucocorticoid for emergency patients, traditional Chinese medicine or proton pump inhibitors for inpatients.CONCLUSION Pharmaceutical administration in Guizhou Province fails to meet the requirement of new index set.A major cause is an acute shortage of pharmaceutical professionals and clinical pharmacists.
