经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果及对切口美观度的影响

Effect of Transumbilical Single Hole Laparoscopy in Myomectomy and Its Influence on Incision Aesthetics

目的:探究经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果及对切口美观度的影响。方法:选取2018年1月-2020年1月本院收治的子宫肌瘤患者80例。根据手术方式不同将其分为观察组和对照组,每组40例。对照组采用传统腹腔镜子宫肌瘤剔除术,观察组采用经脐单孔腹腔镜子宫肌瘤剔除术。比较两组手术时间、术中出血量、术后24 h引流量、术后排气时间、术后下床时间及术后住院时间。比较两组术后即刻和6、24 h的疼痛情况。比较两组术后并发症发生情况。结果:观察组手术时间短于对照组、且术中出血量及术后24 h引流量均少于对照组(P<0.05)。两组术后排气时间、术后下床时间及术后住院时间比较,差异均无统计学的意义(P>0.05)。术后即刻,两组疼痛评分比较,差异无统计学意义(P>0.05);术后6、24 h,观察组疼痛评分均低于对照组(P<0.05)。观察组并发症发生率为5.00%低于对照组的20.00%(P<0.05)。观察组切口美观度评分高于对照组(P<0.05)。结论:经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果较好,可以缩短手术时间,减少术中出血量及术后24 h引流量,加快患者康复进程,且术后疼痛程度较轻,并发症发生率较低,安全性高,切口美观度高,值得临床推广应用。

Objective:To explore the effect of transumbilical single hole laparoscopy in myomectomy and its influence on incision aesthetics.Method:A total of 80 patients with uterine fibroids admitted to our hospital from January 2018 to January 2020 were selected.They were divided into observation group and control group according to different surgical methods,with 40 cases in each group.The control group was treated with traditional laparoscopic myomectomy,and the observation group was treated with transumbilical single hole laparoscopy in myomectomy.The operative time,intraoperative blood loss,24 h postoperative drainage volume,postoperative exhaust time,postoperative time out of bed and postoperative length of hospital stay were compared between the two groups.The pain of the two groups were compared immediately,6 h and 24 h after surgery.The incidence of postoperative complications was compared between the two groups.Result:The operative time in the observation group was shorter than that in the control group,and the amount of intraoperative blood loss and 24 h postoperative drainage volume in the observation group were less than those in the control group (P<0.05).There were no significant differences in the postoperative exhaust time,postoperative time out of bed and postoperative length of hospital stay between the two groups (P>0.05).Immediately after surgery,there was no significant difference in pain scores between the two groups (P>0.05);at 6 and 24 h after surgery,the pain scores of observation group were lower than those of control group (P<0.05).The incidence of complications in the observation group was 5.00%,lower than 20.00% in the control group (P<0.05).The incision aesthetic score of the observation group was higher than that of the control group (P<0.05).Conclusion:The application of transumbilical single hole laparoscopy in myomectomy has a good effect,it can shorten the operation time,reduce the amount of intraoperative bleeding and 24 h drainage after operation,accelerate the recovery process of patients,and the degree of postoperative pain is less,the incidence of postoperative complications is lower,the safety is high,and the incision is beautiful,it is worthy of clinical application.
