脐动脉及大脑中动脉血流指标在胎儿宫内窘迫中的检测价值探究

Investigation on the Detection Value of Umbilical Artery and Middle Cerebral Artery Blood Flow Indexes of Fetus with Intrauterine Distress

目的:探究脐动脉及大脑中动脉血流指标在胎儿宫内窘迫中的检测价值。方法:选取2019年1月-2020年8月于本院就诊的80例胎儿宫内窘迫孕妇为观察组,另选取同时期的80例正常孕妇为对照组。比较两组的脐动脉及大脑中动脉血流指标(S/D、PI及RI),同时比较观察组中不同胎儿生物物理评分者的脐动脉及大脑中动脉血流指标,并采用Spearman秩相关分析脐动脉、大脑中动脉血流指标与胎儿生物物理评分的关系。结果:观察组的脐动脉血流指标均高于对照组,大脑中动脉血流指标均低于对照组,且观察组中胎儿生物物理评分较低者的脐动脉血流指标均高于评分较高者,胎儿生物物理评分较低者的大脑中动脉血流指标均低于评分较高者,差异均有统计学意义(P<0.05)。Spearman秩相关分析显示,脐动脉血流指标与胎儿生物物理评分呈负相关(P<0.05),大脑中动脉血流指标与胎儿生物物理评分呈正相关(P<0.05)。结论:脐动脉及大脑中动脉血流指标在宫内窘迫胎儿中显著异常,且与严重程度密切相关,因此在胎儿宫内窘迫中的检测价值较高。

Objective:To investigate the detection value of umbilical artery and middle cerebral artery blood flow indexes of fetus with intrauterine distress.Method:A total of 80 pregnant women with intrauterine distress admitted to our hospital from January 2019 to August 2020 were selected as the observation group,80 normal pregnant women during the same period were selected as the control group.Then the umbilical artery and middle cerebral artery blood flow indexes (S/D,PI and RI) of two groups were compared,the umbilical artery and middle cerebral artery blood flow indexes of observation group with different fetal biophysical profile scores were compared,then the relationship between umbilical artery,middle cerebral artery blood flow indexes and fetal biophysical profile scores were analyzed with Spearman rank correlation analysis.Result:Umbilical artery blood flow index in observation group were higher than those of the control group,middle cerebral artery blood flow index were lower than those of the control group,fetal biophysical score lower in the observation group of umbilical artery blood flow index were higher than those of score high,fetal biophysical score lower middle cerebral artery blood flow index were lower than those of score high,the differences were statistically significant (P<0.05).The Spearman rank correlation analysis showed the umbilical artery blood flow indexes were negatively correlated with fetal biophysical profile score (P<0.05),the middle cerebral artery blood flow indexes were positively correlated with fetal biophysical profile score (P<0.05).Conclusion:The umbilical artery and middle cerebral artery blood flow indexes of fetus with intrauterine distress are significantly abnormal,they have close relationship to the severity degree,so the detection value in the fetus with intrauterine distress are higher.
