外周血嗜酸性粒细胞与慢性阻塞性肺疾病急性加重期的相关性研究——66例回顾性分析

Study on the correlation between peripheral blood eosinophils and acute exacerbation of chronic obstructive pulmonary disease——Retrospective analysis of 66 cases

目的探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者的外周血嗜酸性粒细胞(PBE)变化情况。方法选择2014年9月至2019年8月住院的AECOPD患者66例,以PBE相对数为2%作为分界值,分为PBE<2%和PBE≥2%两组,观察两组各指标的情况。结果白细胞、中性粒细胞随着PBE升高呈下降趋势。PBE<2%组的白细胞、中性粒细胞、中淋比、降钙素原(PCT)水平相对较高,AECOPD患者的基础状态、有否合并慢性疾病、营养状态等因素对PBE水平的影响可能较少,PBE≥2%组平均住院日缩短,一年内再住院发生率升高,但差异无统计学意义。两组PBE变化率差异无统计学意义,观察2年内反复住院患者的首末次PBE,72.00%患者PBE持续保持在2%以上,61.54%患者持续保持在2%以下,稳定性均高于国外文献报道。结论PBE可作为评估AECOPD病情的炎症指标之一,其变化率与相对数高低无关,AECOPD患者PBE具有一定稳定性。

Objective To investigate the changes of peripheral blood eosinophils(PBE)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 66 patients with AECOPD hospitalized from September 2014 to August 2019 were selected and divided into two groups,PBE<2%and PBE≥2%,with the relative number of PBE being 2%as the cut-off value.The indicators of the two groups were observed.Results The white blood cells and neutrophils decreased with the increase of PBE.The levels of white blood cells,neutrophils,mesochromatin ratio and PCT were relatively higher in PBE<2%group.AECOPD patients'basic state,presence or absence of chronic diseases,nutritional status and other factors may have less influence on PBE level.The average length of stay in the PBE≥2%group was shortened,and the incidence of readmission was increased within one year,but the difference was not statistically significant.There was no significant difference in PBE change rate between the two groups.The first and last PBE of patients with repeated hospitalization within 2 years were observed.The PBE of 72.00%patients and 61.54%patients were kept above 2%and below 2%respectively,both of which were more stable than those reported in foreign literatures.Conclusion PBE can be used as one of the inflammatory indicators to evaluate the condition of AECOPD,and its rate of change has nothing to do with the relative number,indicating that PBE in AECOPD patients is stable to a certain extent.
