氟康唑在霉菌性阴道炎患者中的疗效及对血清、阴道灌洗液中细胞因子的影响研究

Effect of Fluconazole in Patients with Mycotic Vaginitis and Its Influence on Cytokines in Serum and Vaginal Lavage Fluid

目的:探讨氟康唑在霉菌性阴道炎患者中的疗效及对血清、阴道灌洗液细胞因子的影响。方法:选取2018年1月-2020年1月到本院就诊的120例霉菌性阴道炎患者,按照随机数字表法将其分为研究组(氟康唑+常规药物治疗+外洗)和对照组(常规药物治疗+外洗),各60例。对比两组治疗后的临床疗效、临床症状(外阴瘙痒、烧灼感、白带异常、排尿疼痛)消失时间、治疗前后血清及阴道灌洗液中细胞因子水平及复发情况。结果:研究组总有效率为96.67%,高于对照组的83.33%,差异有统计学意义(P<0.05)。研究组外阴瘙痒、烧灼感、白带异常、排尿疼痛等临床症状消失时间均较对照组短,差异均有统计学意义(P<0.001)。治疗后,两组血清和阴道灌洗液中IL-2、IL-10、TNF-α水平均较治疗前改善,且研究组均优于对照组,差异均有统计学意义(P<0.05)。研究组复发率低于对照组,差异有统计学意义(P<0.05)。结论:氟康唑在霉菌性阴道炎治疗中的效果较好,其临床症状消失时间较短,有效缓解患者不适,可改善血清、阴道灌洗液中细胞因子水平,降低炎性反应,且复发率较低,临床应用意义较大。

Objective:To investigate the efficacy of Fluconazole in the treatment of mycotic vaginitis and its effect on cytokines in serum and vaginal lavage fluid.Method:A total of 120 patients with mycotic vaginitis who visited our hospital from January 2018 to January 2020 were selected,they were divided into the study group(Fluconazole+conventional drug therapy+external washing)and the control group(conventional drug therapy+external washing)according to the random number table method,60 cases in each group.The clinical efficacy,the disappearance time of clinical symptoms(vulvae pruritus,burning sensation,abnormality leucorrhea,urination pain),the levels of cytokines in serum and vaginal lavage fluid before and after treatment,the recurrence were compared between two groups.Result:The total effective rate of the study group was 96.67%,it was higher than 83.33%of the control group,the difference was statistically significant(P<0.05).The disappearance time of clinical symptoms such as vulva pruritus,burning sensation,abnormal leucorrhea,urination pain in the study group were all shorter than those in the control group,the differences were statistically significant(P<0.001).After treatment,the levels ofIL-2,IL-10 and TNF-αin serum and vaginal lavage fluid of two groups were improved compared with those of before treatment,the study group were superior to the control group,with statistically significant differences(P<0.05).The recurrence rate of the study group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Fluconazole has a good effect in the treatment of mycotic vaginitis,its clinical symptoms disappear for a short time,which can effectively relieve the discomfort of patients,inhibit the levels of cytokines in serum and vaginal lavage fluid,reduce the inflammatory reaction,the recurrence rate is low,so it is of great significance in clinical application.
