尺骨鹰嘴骨折克氏针张力带和解剖钢板内固定效果对比观察

Effect of Kirschner Wire Tension Band Internal Fixation and Anatomical Plate Internal Fixation for Ulnar Olecranon Fractures

目的:探讨克氏针张力带内固定和解剖钢板内固定治疗尺骨鹰嘴骨折的效果,为相关患者治疗提供参考。方法:选取2012年1月-2019年12月本院收治的60例尺骨鹰嘴骨折患者的诊疗资料进行回顾性分析。按照治疗方式不同将其分钢板组和克氏针组,每组30例。钢板组采用解剖钢板内固定治疗,克氏针组采用克氏针张力带内固定治疗。比较两组手术相关指标、术后3个月治疗效果、肘关节功能及并发症发生情况。结果:两组治疗优良率比较,差异无统计学意义(P>0.05)。钢板组术中出血量与切口长度均高于克氏针组,而手术时间短于克氏针组(P<0.05);两组骨折愈合时间与住院时间比较,差异均无统计学意义(P>0.05)。术后3个月,两组Mayo肘关节评分、伸、屈、旋后功能及前臂旋前活动度明显高于治疗前(P<0.05),但两组间比较,差异均无统计学意义(P>0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:克氏针张力带与解剖钢板内固定治疗尺骨鹰嘴骨折均可取得较满意效果。两种治疗方法各有特点,临床可根据患者具体骨折情况以及术者操作习惯合理选择。

Objective:To investigate the effect of Kirschner wire tension band internal fixation and anatomical plate internal fixation in the treatment of ulnar olecranon fractures,and to provide reference for the treatment of related patients.Method:The diagnosis and treatment data of 60 patients with ulnar olecranon fractures admitted to our hospital from January 2012 to December 2019 were retrospectively analyzed.The patients were divided into plate group and Kirschner wire group according to different treatment methods,with 30 patients in each group.The plate group was treated with anatomical plate internal fixation,and the Kirschner wire group was treated with Kirschner wire tension band internal fixation.Operation-related indicators,treatment effect of 3 months after operation,elbow joint function and complications were compared between the two groups.Result:There was no significant difference in the excellent and good rates between the two groups(P>0.05).The intraoperative blood loss and incision length in the plate group were higher than those in the Kirschner wire group,but the operative time in the plate group was shorter than that in the Kirschner wire group(P<0.05).There were no significant differences in fracture healing time and hospitalization time between the two groups(P>0.05).3 months after surgery,Mayo elbow joint score,extension,flexion,supination function and forearm pronation range of motion in both groups were significantly higher than those before treatment(P<0.05),but there were no statistical significances between the two groups(P>0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Kirschner wire tension band and anatomical plate internal fixation can achieve satisfactory results in the treatment of ulnar olecranon fractures.The two methods have their own characteristics,which can be reasonably selected according to the specific fracture situation of patients and the operation habits of the operators.
