摘要
目的:研究基于奥马哈系统理论的精细化饮食管理应用于食管癌术后患者的效果。方法:选取2018年9月-2019年10月于本院就诊的73例食管癌术后患者,按照随机数字表法将其分为对照组(36例)和研究组(37例)。对照组行常规食管癌术后干预,研究组在对照组的基础上行奥马哈精细饮食干预。观察两组干预后恢复时间、营养指标、免疫指标、术后并发症。结果:研究组肠鸣音恢复时间、肛门排气时间、首次排便时间、下床活动时间均短于对照组,差异均有统计学意义(P<0.05)。干预1、2周后,两组ALB、PAB、Tf水平均较干预前升高,且研究组均高于对照组,差异均有统计学意义(P<0.05)。干预1、2周后,研究组IgM、IgG、IgA水平均升高,且均高于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:奥马哈精细饮食干预可促进患者恢复,改善营养指标,增强免疫功能,改善患者预后,可临床推广。
Objective:To study the effect of refined diet management based on Omaha system theory on patients with esophageal cancer after surgery.Method:A total of 73 patients with esophageal cancer after surgery admitted to our hospital from September 2018 to October 2019 were selected and divided into control group(36 cases)and study group(37 cases),according to random number table method.The control group received routine postoperative intervention for esophageal cancer,the study group received Omaha refined diet management on the basis of the control group.The postoperative recovery time,nutrition index,immune index and postoperative complications after intervention of two groups were observed.Result:The recovery time of bowel sounds,anal exhaust time,first defecation time,and time to get out of bed in the study group were shorter than those in the control group,the differences were statistically significant(P<0.05).After 1 week and 2 weeks of intervention,the levels of ALB,PAB and Tf in the two groups were higher than before the intervention,the study group were higher than the control group,the differences were statistically significant(P<0.05).After 1 week and 2 weeks of intervention,the levels of IgM,IgG and IgA in the study group were all increased,all higher than those in the control group,the differences were statistical significance(P<0.05).The incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Omaha refined diet intervention can promote patient recovery,improve nutritional indexes,enhance immune function,improve patient prognosis,can be clinically promoted.
