缬沙坦对老年高血压患者血压及RAAS指标的影响研究 被引量:2

摘要 目的:探讨缬沙坦对老年高血压患者的血压及肾素-血管紧张素-醛固酮系统(RAAS)指标的影响。方法:选取2019年5月-2020年8月本院收治的100例老年高血压患者为研究对象,将患者随机分为对照组和观察组,每组50例。对照组采用氨氯地平治疗,观察组采用缬沙坦联合氨氯地平治疗。比较两组患者的效果、血压、空腹血糖、胆固醇、肱动脉直径、血管内皮舒张功能(FMD)、肾素(PRA)、血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)、内皮素(ET)以及一氧化氮(NO)水平。结果:观察组患者的总有效率为96.0%,显著高于对照组的72.0%(P<0.05);治疗后,观察组患者的血压、空腹血糖、胆固醇均明显低于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者的肱动脉直径和FMD均显著高于对照组(P<0.05);治疗后,观察组患者的PRA、AngⅡ、NO均明显高于对照组(P<0.05),而ALD、ET均明显低于对照组(P<0.05)。结论:缬沙坦能有效降低老年高血压患者的血压,改善内皮功能,对RAAS指标阻滞更完全。 Objective:To investigate the effect of valsartan on blood pressure and renin angiotensin aldosterone system(RAAS)in elderly patients with hypertension.Method:A total of 100 elderly patients with hypertension in our hospital from May 2019 to August 2020 were selected,they were randomly divided into the control group and observation group,50 cases in each group.The control group was treated with Amlodipine,and the observation group was treated with Valsartan combined with Amlodipine.The effect,blood pressure,fasting blood glucose,cholesterol,brachial artery diameter,flow-mediated dilation(FMD),plasma renin activity(PRA),angiotensinⅡ(AngⅡ),aldosterone(ALD),endothelin(ET)and nitric oxide(NO)levels of the two groups were compared.Result:The total effective rate of the observation group was 96.0%,which was significantly higher than 72.0%of the control group(P<0.05);after treatment,the blood pressure,fasting blood glucose and cholesterol of the observation group were significantly lower than those of the control group,the differences were statistically significant(P<0.05);after treatment,the brachial artery diameter and FMD of the observation group were significantly higher than those of the control group(P<0.05);after treatment,PRA,AngⅡand NO in the observation group were significantly higher than those in the control group(P<0.05),while ALD and ET in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:Valsartan can effectively reduce blood pressure,improve endothelial function and block RAAS index in elderly patients with hypertension.
