探讨黄体期长方案HCG日LH水平对体外受精胚胎移植妊娠结局的临床效果

Exploration on the Clinical Effect of LH Level on HCG Day of Long Luteal Phase Regimen on Pregnancy Outcome of in Vitro Fertilization Embryo Transfer

目的分析黄体期长方案HCG日LH水平对体外受精胚胎移植妊娠结局的临床效果。方法该研究便利选取2018年1—12月148例体外受精/胚胎移植患者进行观察,监测患者HCG日LH水平,并按照结果分组,观察组HCG日LH未超过2.0 mIU/mL,对照组HCG日LH超过2.0 mIU/mL,患者均应用黄体期长效长方案助孕,对比两组的妊娠结局以及临床指标。结果观察组患者Gn天数和总量分别是(12.45±1.33)d、(2474.65±527.38)IU,HCG日E2和P分别是(4263.75±836.83)pg/mL、(1.12±0.32)ng/mL,数据较之对照组有差异有统计学意义(t=11.570、3.791、4.093、5.376,P<0.05);两组获卵数、可利用胚胎以及平均移植胚胎数差异无统计学意义(t=0.154、0.175、0.476,P>0.05);观察组妊娠率为62.16%(46例),较之对照组差异有统计学意义(χ^(2)=5.313,P<0.05),流产率差异无统计学意义(χ^(2)=0.027,P>0.05)。结论黄体期长效长方案HCG日LH水平未超过2.0 mIU/mL时,患者妊娠结局较好,各项孕期指标较为理想,超过2.0 mIU/mL则尽量进行全胚冷冻。

Objective To analyze the clinical effect of the LH level on the HCG day of the long luteal phase regimen on the pregnancy outcome of in vitro fertilization embryo transfer.Methods Convenient select The study took 148 patients with in vitro fertilization/embryo transfer for observation.The enrollment period was from January to December 2018.The HCG day LH level of the patients was monitored and analyzed according to the results.The observation group HCG day LH did not exceed 2.0mIU/ml In the control group,the HCG day LH exceeded 2.0mIU/ml,and the patients were all treated with the luteal phase long-acting long-acting program to assist in pregnancy.The pregnancy outcome and clinical indicators of the two groups were compared.Results The number of days and total amount of Gn in the observation group were(12.45±1.33)d,(2474.65±527.38)IU,and E2 and P on HCG day were(4263.75±836.83)pg/mL and(1.12±0.32)ng/mL,Compared with the control group,the difference was statistically significant(t=11.570,3.791,4.093,5.376,P<0.05);the two groups had no significant difference in the number of eggs,available embryos and the average number of embryos transferred(t=0.154,0.175,0.476,P>0.05);the pregnancy rate in the observation group was 62.16%(46 cases),which was statistically significant compared to the control group(χ^(2)=5.313,P<0.05),and the abortion rate was not statistically significant(χ^(2)=0.027,P>0.05).Conclusion When the HCG day LH level of the luteal phase long-acting long-term regimen does not exceed 2.0 mIU/ml,the patient's pregnancy outcome is better,and the various pregnancy indicators are relatively ideal.If it exceeds 2.0 mIU/mL,try to freeze the whole embryo.
