摘要
目的探究重症肺炎呼吸衰竭患者给予序贯机械通气治疗的临床应用效果。方法分组研究方便选取该院自2015年2月—2020年2月接收104例重症肺炎呼吸衰竭患者,对照组(52例)患者行常规机械通气治疗,观察组(52例)患者行序贯机械通气治疗,先给予有创通气治疗,再给予无创通气治疗。比较两组患者机械通气治疗后各临床指标、治疗前后血气分析指标情况。结果观察组经序贯机械通气治疗,与对照组相比,患者通气时间(7.3±3.8)d、总通气时间(10.9±4.5)d、ICU住院时间(16.1±3.9)d,观察组所用时间明显变短,差异有统计学意义(t=5.354、6.854、8.624,P<0.05)。观察组经序贯机械通气治疗,相比于对照组,患者PaO2(80.03±7.51)mmHg、PaCO2(33.78±8.46)mmHg,组间差异有统计学意义(t=3.254、6.252,P<0.05)。结论重症肺炎呼吸衰竭患者给予序贯机械通气治疗,既有利于改善患者血流动力学、缩短患者通气及治疗时间,又有利于改善患者呼吸功能及预后效果,对患者生活质量具有重大提升作用。
Objective To explore the clinical application effect of sequential mechanical ventilation in patients with severe pneumonia and respiratory failure.Methods Group study The hospital received 104 patients with severe pneumonia and respiratory failure in the hospital from February 2015 to February 2020.The control group(52 cases)received conventional mechanical ventilation treatment,and the observation group(52 cases)received sequential mechanical ventilation treatment.First give invasive ventilation treatment,then give non-invasive ventilation treatment.The clinical indicators and blood gas analysis indicators before and after treatment were compared between the two groups of patients after mechanical ventilation treatment.Results The observation group was treated with sequential mechanical ventilation.Compared with the control group,the patient's ventilation time was(7.3±3.8)d,total ventilation time(10.9±4.5)d,ICU hospitalization time(16.1±3.9)d,and the time spent in the observation group was significant Shortened,the difference was statistically significant(t=5.354,6.854,8.624,P<0.05).The observation group was treated with sequential mechanical ventilation.Compared with the control group,PaO2(80.03±7.51)mmHg and PaCO2(33.78±8.46)mmHg were significantly different between the groups(t=3.254,6.252,P<0.05).Conclusion Sequential mechanical ventilation therapy for patients with severe pneumonia and respiratory failure can not only improve the patient's hemodynamics,shorten the patient's ventilation and treatment time,but also improve the patient's respiratory function and prognosis,and has a significant effect on the patient's quality of life.
