交锁髓内钉早期动力化对于创伤性骨折愈合的促进作用

The Effect of Early Dynamization of Interlocking Intramedullary Nails on the Healing of Traumatic Fractures

目的探讨交锁髓内钉早期动力化对于创伤性骨折愈合的促进作用。方法方便选择2018年12月—2019年11月该院收治的创伤性胫骨骨折患者106例,以随机数字表法将其分为两组,其中对照组与研究组各53例。对照组采取常规交锁髓内钉治疗,研究组采取交锁髓内钉早期动力化治疗。对比两组治疗的优良率、术前与术后6个月时骨谷氨酸蛋白(BGP)与骨保护素(OPG)的水平、骨性愈合时间、术后并发症情况。结果研究组治疗的优良率98.11%高于对照组84.91%,差异有统计学意义(χ^(2)=4.371,P<0.05)。术后6个月时,研究组BGP(23.52±3.52)μg/L、OPG(16.52±1.52)pmol/L水平均高于对照组(17.36±2.45)μg/L、(13.22±1.08)pmol/L,差异有统计学意义(t=10.457、12.884,P<0.001)。研究组骨性愈合时间(7.52±0.48)d低于对照组(13.15±0.45)d,差异有统计学意义(t=19.928,P<0.001)。研究组术后并发症的发生率1.89%低于对照组15.09%,差异有统计学意义(χ^(2)=4.371,P<0.05)。结论交锁髓内钉早期动力化在创伤性骨折患者中具有显著的应用效果与安全性,且对于骨折愈合具有显著的促进作用。

Objective To investigate the effect of early dynamization of interlocking intramedullary nails on the healing of traumatic fractures.Methods A total of 106 patients with traumatic tibial fractures admitted to the hospital from December 2018 to November 2019 were conveniently selected and divided into two groups by random number table method,with 53 cases in the control group and 53 in the study group.The control group took conventional interlocking intramedullary nail treatment,and the study group took interlocking intramedullary nail early dynamic treatment.The excellent and good rate of treatment,the levels of bone glutamate(BGP)and osteoprotegerin(OPG)before and 6 months after operation,bone healing time,and postoperative complications were compared between the two groups.Results The excellent and good rate of treatment in the study group was 98.11%higher than that in the control group 84.91%(χ^(2)=4.371,P<0.05).At 6 months after surgery,the levels of BGP(23.52±3.52)μg/L and OPG(16.52±1.52)pmol/L in the study group were higher than those in the control group(17.36±2.45)μg/L,(13.22±1.08)pmol/L(t=10.457,12.884,P<0.001).The bone healing time of the study group was(7.52±0.48)d lower than that of the control group(13.15±0.45)d(t=19.928,P<0.001).The incidence of postoperative complications in the study group was 1.89%lower than that in the control group 15.09%(χ^(2)=4.371,P<0.05).Conclusion Early dynamization of interlocking intramedullary nails has significant application effect and safety in patients with traumatic fractures,and it has a significant role in promoting fracture healing.
