探讨循环肿瘤细胞(CTCs)在乳腺癌患者预后判断中的应用价值

Exploration on the Application Value of Circulating Tumor Cells(CTCs)in the Prognosis of Breast Cancer Patients

目的分析循环肿瘤细胞(CTCs)在乳腺癌患者预后判断中的应用价值。方法方便选取2017年1月—2019年2月374例乳腺癌患者,所有患者行FEC化疗方案治疗。统计患者的临床疗效,同时比较不同化疗效果患者化疗前后CTCs计数的差异,并采用Spermans相关分析法分析CTCs与乳腺癌化疗效果的相关性。结果该次研究中374例患者中经化疗治疗后,完全缓解105例,部分缓解141例,病情稳定68例,病情进展60例。完全缓解化疗前后CTCs计数为(6.5±0.6)个和(1.2±0.2)个,部分缓解化疗前后CTCs计数为(7.1±0.8)个和(2.0±0.4)个,病情稳定化疗前后CTCs计数为(11.8±2.1)个和(4.6±1.1)个,病情进展化疗前后CTCs计数为(12.1±2.3)个和(5.6±1.4)个,完全缓解与部分缓解患者化疗前后的CTCs计数相比病情稳定和病情进展患者明显更低,差异有统计学意义(t=6.652、5.263、5.165、5.024,P=0.046、0.042、0.041、0.040)。通过Spermans相关分析法分析,CTCs计数与乳腺癌患者化疗效果呈正相关性(r=0.426,P<0.05)。结论化疗前CTCs计数低的乳腺癌患者往往预后较好,且化疗后乳腺癌患者CTCs明显下降,循环肿瘤细胞在乳腺癌患者预后判断中具有较好的应用价值,可在临床推广使用。

Objective To analyze the application value of circulating tumor cells(CTCs)in the prognosis of breast cancer patients.Methods A total of 374 breast cancer patients from January 2017 to February 2019 were conveniently selected,and all patients were treated with FEC chemotherapy.The clinical efficacy of patients was counted,and the differences in CTCs counts before and after chemotherapy in patients with different chemotherapy effects were compared.The Spermans correlation analysis method was used to analyze the correlation between CTCs and chemotherapy effects of breast cancer.Results Of the 374 patients in this study,after chemotherapy treatment,105 cases were completely relieved,141 cases were partially relieved,68 cases were stable,and 60 cases progressed.The counts of CTCs before and after complete remission chemotherapy were(6.5±0.6)and(1.2±0.2),the counts of CTCs before and after partial remission chemotherapy were(7.1±0.8)and(2.0±0.4),and the counts of CTCs before and after chemotherapy in stable condition were(11.8±2.1)and(4.6±1.1).The counts of CTCs before and after chemotherapy were(12.1±2.3)and(5.6±1.4).Complete remission and partial remission were stable and stable compared with the CTCs count before and after chemotherapy.Patients with progressive disease were significantly lower,the difference was statistically significant(t=6.652,5.263,5.165,5.024,P=0.046,0.042,0.041,0.040).Through the analysis of Spermans correlation analysis,the count of CTCs was positively correlated with the chemotherapy effect of breast cancer patients(r=0.426,P<0.05).Conclusion Breast cancer patients with low CTCs counts before chemotherapy often have a better prognosis,and CTCs in breast cancer patients significantly decrease after chemotherapy.Circulating tumor cells have good application value in the prognosis of breast cancer patients and can be used in clinical promotion.
