摘要
目的探讨超声用于剖宫产瘢痕妊娠临床诊断的价值。方法便利选择该院2017年1月—2020年3月收治的50例剖宫产瘢痕妊娠产妇为研究对象,选择同期正常子宫妊娠50例孕妇作为对照,对比两组超声检查结果。结果研究组孕妇肌层厚度(4.04±1.14)mm低于对照组(8.52±2.38)mm,差异有统计学意义(t=6.12,P<0.05),RI值(0.52±0.12)低于对照组(0.68±0.13),差异有统计学意义(t=7.33,P<0.05)。所有瘢痕妊娠孕妇的妊娠囊周边均存在来自子宫切口肌层的血流信号。按照子宫瘢痕妊娠的CDFI图像对其进行分型,则Ⅰ型、Ⅱ型、Ⅲ型的产妇分别14例(28.00%)、27例(54.00%)、9例(18.00%),以Ⅲ型产妇血流信号最为丰富达77.78%。结论超声用于剖宫产瘢痕妊娠临床诊断的价值高,可以依据妊娠囊、肌层厚度、血供等情况进行分型指导临床治疗方案的制定以及预测产妇预后,具有重要的应用意义。
Objective To explore the value of ultrasound in clinical diagnosis of cesarean section scar pregnancy.Methods 50 pregnant women with cesarean section scars admitted to the hospital from January 2017 to March 2020 were conveniently selected as the research objects,50 pregnant women with normal uterine pregnancy during the same period were selected as controls,and the ultrasound examination results of the two groups were compared.Results The thickness of the muscular layer of pregnant women in the study group was(4.04±1.14)mm lower than that of the control group(8.52±2.38)mm,the difference was statistically significant(t=6.12,P<0.05),and the RI value was(0.52±0.12)lower than that of the control group(0.68±0.13),the difference was statistically significant(t=7.33,P<0.05).All pregnant women with scar pregnancy had blood flow signals from the muscle layer of the uterine incision around the gestational sac.According to the CDFI image of uterine scar pregnancy,14 cases(28.00%),27 cases(54.00%),9 cases(18.00%)were classified as type Ⅰ,type Ⅱ,and type Ⅲ.The blood flow signal was the most abundant up to 77.78%.Conclusion Ultrasound is of high value in the clinical diagnosis of cesarean scar pregnancy.It can be classified based on the gestational sac,muscle thickness,blood supply and other conditions to guide the formulation of clinical treatment plans and predict maternal prognosis,which has important application significance.
