自主呼吸静脉麻醉下胸腔镜治疗纵隔肿瘤的临床效果

Clinical Effect of Thoracoscopic Treatment of Mediastinal Tumors under Spontaneous Breathing and Intravenous Anesthesia

目的探析自主呼吸静脉麻醉下胸腔镜(VATS)治疗纵隔肿瘤的临床效果。方法于2016年12月—2019年12月在该院就诊的纵隔肿瘤患者中随机选取100例,随机分为两组,每组50例,100例患者均接受VATS治疗,麻醉时,对照组选择双腔气管插管常规静脉吸入复合麻醉,观察组选择自主呼吸静脉麻醉,对比麻醉的效果。结果两组术后4 h肌力、麻醉插管时间、发生并发症的发生率对比,观察组(38.4±6.6)kg、(7.7±4.9)min、18.0%优于对照组[(28.2±5.3)kg、(18.5±8.18)min、36.0%],差异有统计学意义(t=4.071、8.009,χ^(2)=4.110,P<0.05);两组住院时间、拔管时间、术后24 h的疼痛评分、术后24 h的肌力、手术时间、出血量对比,差异无统计学意义(P>0.05)。结论以VATS治疗纵隔肿瘤时选择自主呼吸静脉麻醉,疗效确切,值得应用。

Objective To explore the clinical effect of thoracoscopy(VATS)under spontaneously breathing intravenous anesthesia in the treatment of mediastinal tumors.Methods 100 cases of mediastinal tumor patients in our hospital were conveniently selected and randomly divided into two groups from December 2016 to December 2019,50 cases in each group,100 cases were treated with VATS.During anesthesia,the control group chose double-lumen endotracheal intubation and conventional intravenous inhalation combined anesthesia,the observation group chose spontaneous breathing intravenous anesthesia to compare the effects of anesthesia.Results Comparison of muscle strength,anesthesia intubation time,and the complication rate between the two groups after 4 hours.The observation group(38.4±6.6)kg,(7.7±4.9)min,18.0%were better than the control group[(28.2±5.3)kg,(18.5±8.18)min,36.0%],the difference were statistically significant(t=4.071,8.009,χ^(2)=4.110,P<0.05);hospitalization time,extubation time,pain score at 24 h after surgery,the comparison of muscle strength,operation time,and blood loss at 24 hours after operation was statistically significant(P>0.05).Conclusion The choice of spontaneously breathing intravenous anesthesia for the treatment of mediastinal tumors with VATS is effective and worthy of application.
