胃肠超声助显剂在超声诊断胆总管中下段病变中应用可提高显示率和诊断符合率

The Application of Gastrointestinal Ultrasound Aids in the Ultrasound Diagnosis of Common Bile Duct Lesions in the Middle and Lower Segments Able to Improve the Display Rate and Diagnosis Coincidence Rate

目的解析胃肠超声助显剂在胆总管中下段病变超声诊断中的作用。方法方便选取该院2018年1月—2019年10月间的胆总管中下段病变患者,共60例,计算机筛选分为对照和观察两组,各30例,前组直接接受超声诊断,后组在超声前口服胃肠超声助显剂,以手术和病理检查结果作为“金标准”,对比诊断情况。结果胆总管中下段肿瘤检出率[观察组(73.33%)>对照组(60.00%)]、胆总管中下段结石[观察组(10.00%)>对照组(3.33%)]、十二指肠壶腹段肿瘤[观察组(6.67%)>对照组(3.33%)];观察组诊出率显著高于对照组[90.00%(27/30)>66.67%(20/30)],差异有统计学意义(χ^(2)=4.812,P=0.028<0.05);观察组诊符合率显著高于对照组[89.66%(26/29)>62.07%(18/29)],差异有统计学意义(χ^(2)=5.455,P=0.020<0.05)。结论胆总管中下段病变诊断管理中,胃肠超声助显剂应用价值高,可提高诊断的准确率。

Objective To analyze the role of gastrointestinal ultrasound aids in the ultrasound diagnosis of diseases of the middle and lower common bile ducts.Methods A total of 60 patients with middle and lower common bile duct diseases in the hospital from January 2018 to October 2019 were conveniently selected.Computer screening was divided into control and observation two groups,30 cases in each group.The former group received ultrasound diagnosis directly,and the latter group adopted oral gastrointestinal ultrasound aids before ultrasound,and the results of surgery and pathological examination were used as the"gold standard"to compare the diagnosis.Results The detection rate of tumors in the middle and lower part of the common bile duct[observation group(73.33%)>control group(60.00%)],stones in the middle and lower part of the common bile duct[observation group(10.00%)>control group(3.33%)],duodenum ampullary tumor[observation group(6.67%)>control group(3.33%)];the diagnosis rate of observation group was significantly higher than that of control group[90.00%(27/30)>66.67%(20/30)],and the difference was statistically significant(χ^(2)=4.812,P=0.028<0.05);the diagnosis coincidence rate of the observation group was significantly higher than that of the control group[89.66%(26/29)>62.07%(18/29)],and the difference was statistically significant(χ^(2)=5.455,P=0.020<0.05).Conclusion In the diagnosis and management of the middle and lower common bile duct lesions,gastrointestinal ultrasound aids have high application value,which can improve the accuracy of diagnosis.
