单核细胞趋化蛋白-1、正五聚蛋白3与脑小血管病认知障碍的相关性研究 被引量:4

Correlation between MCP-1,PTX3 and cognitive disorder in cerebral small vessel disease

摘要 目的探究单核细胞趋化蛋白-1(MCP-1)、正五聚蛋白3(PTX3)与脑小血管病(CSVD)患者认知功能障碍的关系,为CSVD患者认知功能障碍的临床防治提供新思路。方法连续纳入CSVD患者261例,采用简易精神状态检查量表评估患者的认知功能,将CSVD患者分为认知正常组(107例,27~30分)和认知障碍组(154例,<27分),认知障碍组又分为轻度认知障碍组(71例,21~26分)、中度认知障碍组(59例,10~20分)和重度认知障碍组(24例,0~9分)。采用ELISA法测定血MCP-1、PTX3的质量浓度,分析上述炎症因子CSVD患者认知功能障碍的相关性。结果(1)认知障碍组的MCP-1、PTX3质量浓度高于认知正常组,差异有统计学意义(P<0.001)。(2)中度和重度认知障碍组的MCP-1、PTX3质量浓度均高于轻度认知障碍组,差异有统计学意义(P<0.001);重度认知障碍组的MCP-1、PTX3质量浓度高于中度认知障碍组,差异有统计学意义(P<0.001)。(3)CSVD患者认知功能障碍程度与血MCP-1、PTX3的质量浓度呈正相关关系(r=0.687,P<0.001;r=0.758,P<0.001)。结论MCP-1、PTX3质量浓度升高与CSVD患者认知障碍及其严重程度密切相关,可作为CSVD患者认知功能障碍的风险评估指标,有利于及早防治血管性认知损害。 Objective To investigate into the correlation between monocyte chemoattractant proteins-1(MCP-1),pentraxin 3(PTX3)and cognitive disorder in patients with cerebral small vessel diseases(CSVD),and to provide new ideas for clinical prevention and treatment of cognitive disorder in patients with cerebral small vessel disease.Methods 261 patients with CSVD were continuously selected.Using Mini Mental State Examination to assess cognitive function,CSVD patients were divided into the cognitive normal group(107 cases,27~30 points)and the cognitive disorder group(154 cases,<27 points);the cognitive disorder group was fur-ther divided into mild cognitive disorder group(71 cases,21~26 points),moderate cognitive disorder group(59 cases,10~20 points)and severe cognitive disorder group(24 cases,0~9 points).The concentrations of MCP-1 and PTX3 in blood were determined by ELISA to analyze the correlation between the above inflammatory factors and cognitive disorder in patients with CSVD.Results(1)The levels of MCP-1 and PTX3 in the cognitive disorder group were significantly higher than the cognitive normal group and had statistically significant(P<0.001).(2)The levels of MCP-1 and PTX3 in moderate and severe cognitive disorder groups were significantly higher than in the mild cognitive disorder group and had statistically significant(P<0.001).The levels of MCP-1 and PTX3 in the group with severe cognitive disorder were higher than those in the group with moderate cognitive disorder and had statistically significant(P<0.001).(3)The degree of cognitive disorder in patients with CSVD was positively correlated with the levels of MCP-1 and PTX3(r=0.687,P<0.001;r=0.758,P<0.001).Conclusion This study showed that increased levels of MCP-1 and PTX3 are closely correlated with cognitive disorder and its severity in patients with CSVD,which can be used as risk assessment indicators for cognitive disorder in patients with CSVD,and is conducive to early prevention and treatment of vascular cognitive impairment.
