传统外侧“L”形切口钢板内固定与跗骨窦小切口微型钢板内固定治疗跟骨骨折患者的效果

Effect of traditional lateral“L”incision plate internal fixation and tarsal sinus small incision mini plate internal fixation in the treatment of calcaneal fractures

目的比较传统外侧“L”形切口钢板内固定与跗骨窦小切口微型钢板内固定治疗跟骨骨折患者的效果。方法选取福建医科大学附属三明第一医院2019年1—12月收治的59例跟骨骨折患者,随机分为试验组(31例)和对照组(28例)。对照组采取外侧“L”形切口钢板内固定治疗,试验组采取跗骨窦小切口微型钢板内固定治疗,比较两组的治疗效果。结果试验组手术时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);两组骨折愈合时间比较,差异无统计学意义(P>0.05)。术后,两组跟骨结节关节角、Gissane角均大于术前,差异有统计学意义(P<0.05);但组间跟骨结节关节角、Gissane角比较,差异无统计学意义(P>0.05)。试验组并发症发生率低于对照组,Maryfand评分、足部功能恢复优良率均高于对照组,差异有统计学意义(P<0.05)。结论跗骨窦小切口微型钢板内固定治疗跟骨骨折患者的效果优于传统外侧“L”形切口钢板内固定,可有效促进患者足部功能恢复,且安全性较高。

Objective To compare the effect of traditional lateral“L”incision plate internal fixation and tarsal sinus small incision mini plate internal fixation in the treatment of calcaneal fractures.Methods 59 patients with calcaneal fracture admitted to Sanming First Hospital Affiliated to Fujian Medical University from January to December 2019 were randomly divided into experimental group(31 cases)and control group(28 cases).The control group was treated with lateral“L”incision plate internal fixation,and the experimental group was treated with tarsal sinus small incision micro plate internal fixation.Results The operation time and hospital stay of the experimental group were shorter than those of the control group,and the amount of blood loss was less than that of the control group,with statistical significance(P<0.05).The fracture healing time of the two groups had no statistical significance(P>0.05).After operation,the joint angle and gissane angle of calcaneal nodule in the two groups were greater than those before operation,and the difference was statistically significant(P<0.05).But there was no significant difference between the two groups(P>0.05).The incidence of complications in the experimental group was lower than that in the control group,the maryfand score and the excellent and good rate of foot function recovery in the experimental group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of mini plate internal fixation through tarsal sinus small incision in the treatment of calcaneal fracture is better than that of traditional lateral“L”incision plate internal fixation,which can effectively promote the recovery of foot function,and has high safety.
