末端瓣膜式高压注射型PICC导管在胃肠道肿瘤患者管理中的应用研究

Application of end valve type high pressure injection PICC catheter in the management of gastrointestinal cancer patients

目的探讨末端瓣膜式高压注射型PICC导管在胃肠道肿瘤患者管理中的应用。方法选择2016年3月-2020年2月期间于本院就诊的328例普外科胃肠道肿瘤术后化疗患者进行分析。采用数字随机法分为两组,各164例,研究组患者采用末端瓣膜式高压注射型PICC导管,参照组患者采用高压注射型PICC导管,分别对比两组患者并发症发生率、置管舒适度、留置时间及导管维护时间。结果两组患者置管后血栓及并发症总发生率对比差异具有统计学意义(P<0.05);两组患者置管维护时间、留置时间、穿刺次数、穿刺静脉及置管史对比差异具有统计学意义(P<0.05);穿刺静脉、穿刺次数及置管史是影响PICC置管患者血栓形成的危险因素(P<0.05)。结论末端瓣膜式高压注射型PICC导管有效降低并发症、缩短置管及维护时间,患者接受度较高,但血管选择、穿刺次数、置管史是影响患者PICC置管血栓形成的危险因素,在普外科胃肠道肿瘤术后化疗患者中推广运用同时加强PICC置管管理。

Objective To investigate the application of end valve type high pressure injection PICC catheter in the management of gastrointestinal cancer patients.Methods 328 cases of postoperative chemotherapy of gastrointestinal tumor in general surgery department from March 2016 to February 2020 in our hospital were selected and randomly divided into two groups,164 cases in each group.The patients in the study group were treated with end valve type high pressure injection type PICC catheter,while the control group were treated with conventional high pressure resistant injection type PICC catheter Indwelling time and catheter maintenance time.Results There were significant differencesin the total incidence of thrombosis and complications between the two groups(P<0.05);there were significant differ-ences in the maintenance time,indwelling time,puncture times,puncture vessels and catheterization history betweenthe two groups(P<0.05);puncture vessels,puncture times and catheterization history were the risk factors of thrombo-sis in PICC patients(P<0.05).Conclusion The end valve type high-pressure injection PICC catheter can effectivelyreduce the complications,shorten the catheterization and maintenance time,and the patients have a high acceptance.It can be further applied in the postoperative chemotherapy of gastrointestinal cancer patients in general surgery.
