摘要
目的通过生物电阻抗法了解肿瘤住院患儿和幸存者的体成分变化。方法选取2019年3月—2020年3月在首都医科大学附属北京儿童医院住院治疗和停止治疗定期复查的134例肿瘤患儿(包括治疗组74例,幸存组60例)及健康对照组。用问卷调查了解三组人群的活动及屏幕使用时间等情况;运用生物电阻抗法行人体成分检测和营养评估。结果治疗组蛋白质量、肌肉量及去脂体重均低于对照组(t值分别为-3.272、-3.221和-3.013,P值均<0.05),体脂百分比高于对照组(t=3.192,P<0.05)。幸存组蛋白质量、肌肉量及去脂体重与对照组相比无统计学差异,而体脂肪量和体脂百分比均高于对照组(t值分别为2.918和6.043,P值均<0.05)。治疗组营养不良发生率高于对照组和幸存组(t值分别为5.325和-3.866,P值均<0.05),超重肥胖发生率和对照组相比无统计学差异。幸存组营养不良和超重肥胖发生率均高于对照组(t值分别为2.053和2.92,P值均<0.05)。结论住院患儿及幸存者均存在体成分的异常,体脂肪显著增加,结果提示有必要为肿瘤儿童制定一个可行的运动和营养的干预计划,预防脂肪的过度增加和肥胖。
Objective Study on body composition changes of hospitalized children and survivors of solid tumor by Bioelectrical impedance analysis.Methods From March 2019 to March 2020,134 children with solid tumor who were hospitalized and reviewed regularly in Beijing Children′s Hospital Affiliated to Capital Medical University(including 74 cases in the treatment group and 60 cases in the survival group)and the healthy control group cases were selected.Using questionnaire to understand the activity and screen use time of three groups of people,detection of human body composition and nutritional evaluation by Bioelectrical impedance analysis.Results The protein,muscle mass and body weight of the treatment group were lower the control group,the differences were statistically significant(t=-3.272、-3.221、-3.013,P<0.05),the percentage of body fat was higher than that of the control group(t=3.192,P<0.05).There was no significant difference in protein,muscle mass and fat free weight between the surviving group and the control group,but the percentage of body fat and body fat was higher than the control group(t=2.918、6.043,P<0.05).The incidence of malnutrition in the treatment group was higher than that in the control group and the survival group(t=5.325、-3.866,P<0.05).The incidence of malnutrition,overweight and obesity in the survival group was higher than that in the control group(t=2.053、2.92,P<0.05).Conclusions There are abnormal body composition and significant increase of body fat in hospitalized children and survivors,the results suggest that it is necessary to make a feasible intervention plan for children with cancer,to prevent the excessive increase of fat and obesity.
