丙泊酚联合去氧肾上腺素诱导在眼科全身麻醉手术中的应用价值

Application Value of Propofol Combined with Norepinephrine Induction in Ophthalmic Surgery Under General Anesthesia

目的:分析丙泊酚联合不同药物诱导在眼科全身麻醉手术中的应用价值。方法:选取2019年7月至2020年7月郑州市第二人民医院收治的全身麻醉喉罩通气下行眼科手术的72例患者,按随机数字表法分为观察组(n=36)与对照组(n=36),观察组采用丙泊酚联合去氧肾上腺素,对照组采用丙泊酚联合顺式阿曲库铵。对比两组患者麻醉优良率、恢复情况(睁眼时间、清醒时间)、不良反应发生率和麻醉诱导前(T1)、诱导后5 min(T2)、手术30 min(T3)及术后10 min(T4)血流动力学[收缩压(SBP)、平均动脉压(MAP)]。结果:观察组麻醉优良率94.44%较对照组77.78%高(P<0.05);T2、T3观察组SBP、MAP水平较对照组低,且观察组波动幅度较对照组小(P<0.05);观察组清醒时间、语言功能恢复时间较对照组短(P<0.05);观察组不良反应发生率5.56%与对照组13.89%比较,差异无统计学意义(P>0.05)。结论:采用丙泊酚联合去氧肾上腺素诱导在眼科全麻手术中麻醉效果显著,可维持血流动力学稳定,缩短术后清醒时间、语言功能恢复时间,不良反应发生率低,值得临床推广应用。

Objective:To analyze the use of propofol compounded with different drugs induction in ophthalmic general anesthesia surgery.Methods:From July 2019 to July 2020,72 patients undergoing ophthalmic surgery with laryngeal mask airway under general anesthesia in Zhengzhou Second People's Hospital were randomly divided into observation group(n=36)and control group(n=36).The observation group was treated with propofol combined with norepinephrine,and the control group was treated with propofol combined with cisatracurium.The excellent and good rate of anesthesia,recovery(eye opening time,awake time),incidence of adverse reactions and hemodynamics[systolic blood pressure(SBP),mean arterial pressure(MAP)]before anesthesia induction(T1),5 min after induction(T2),30 min after operation(T3)and 10 min after operation(T4)were compared between the two groups.Results:The excellent and good rate of anesthesia in the observation group was 94.44%,which was higher than 77.78%in the control group(P<0.05);the levels of SBP and map in the T2 and T3 observation groups were lower than those in the control group,and the fluctuation amplitude of the observation group was smaller than that in the control group(P<0.05);the recovery time of awake time and language function in the observation group was shorter than that in the control group(P<0.05);the incidence of adverse reactions in the observation group was 5.56%,which was not statistically significant compared with 13.89%in the control group(P>0.05).Conclusions:The use of propofol compounded with phenylephrine to induce anaesthesia in ophthalmic general anaesthesia surgery resulted in a remarkable anaesthetic effect,which could maintain haemodynamic stability,reduce the time of postoperative wakefulness and recovery of language function,with a low incidence of adverse effects,and merits clinical promotion.
