中药联用外治法治疗儿童腺样体肥大临床疗效

Clinical Efficacy of Traditional Chinese Medicine in Treating Child Adenoid Hypertrophy

目的:探究散结化痰缩腺方联用外治法治疗儿童腺样体肥大的临床疗效。方法:选取符合标准的90例儿童腺样体肥大病例随机分为对照组(内服孟鲁司特钠)、中药内治组(内服散结化痰缩腺方)、内外合治组(内服散结化痰缩腺方+穴位按摩),每组各30例,疗程为2月。结果:疾病总疗效、中医证候疗效比较,内外合治组均明显优于对照组(P<0.01),中医内治组均优于对照组(P<0.05)。主要症状积分(鼻塞程度、打鼾程度、张口呼吸、睡眠质量)治疗后均明显优于治疗前(P<0.01),且治疗后内外合治组、中医内治组各主症均明显优于对照组(P<0.01)。各组次要症状积分治疗前后均有统计学差异(P<0.05)。在鼻流涕、腺样体面容方面,内外合治组明显优于对照组(P<0.01)。在盗汗自汗、感冒次数、中耳炎次数方面,内外合治组优于对照组(P<0.05)。在鼻流涕方面,中医内治组明显优于对照组(P<0.01)。在盗汗自汗、感冒次数方面,中医内治组优于对照组(P<0.05)。在腺样体面容方面,内外合治组优于中医内治组(P<0.05)。腺样体大小与症状总积分正相关(P<0.05),症状总积分与生活质量总积分均正相关(P<0.05)。结论:散结化痰缩腺方结合穴位按摩操内外合治能提高儿童腺样体肥大的临床疗效,明显改善患儿的生活质量,其疗效优于西药组。

Objective:To explore the clinical effects of Sanjie Huatan Shuoxian decoction combined with external therapy in the treatment of adenoid hypertrophy in children.Methods:Ninety cases of adenoid hypertrophy in children meeting the criteria were randomly divided into a control group (orally administered montelukast sodium),a traditional Chinese medicine treatment group (orally taken Sanjie Huatan Shuoguan prescription),and an internal and external combined treatment group (orally taken powder) Sanjie Huatan Shuoxian decoction + Acupoint Massage),30 cases in each group,the course of treatment is 2 months.Results:Comparing the total curative effect of the disease and the curative effect of TCM syndromes,the internal and external treatment group was significantly better than the control group (P<0.01),and the TCM internal treatment group was better than the control group (P<0.05).The main symptom scores (degree of nasal congestion,degree of snoring,mouth breathing,quality of sleep) after treatment were significantly better than those before treatment (P<0.01),and the main symptoms of the internal and external treatment group and the internal Chinese medicine treatment group were significantly better than the control after treatment Group (P<0.01).The scores of minor symptoms in each group were statistically different before and after treatment (P<0.05).In terms of nasal discharge and adenoid facial appearance,the internal and external treatment group was significantly better than the control group (P<0.01).In terms of night sweats and spontaneous sweats,colds,and otitis media,the internal and external treatment group was better than the control group (P<0.05).In terms of nasal runny nose,the TCM internal treatment group was significantly better than the control group (P<0.01).In terms of night sweats,spontaneous sweats,and colds,the TCM internal treatment group was better than the control group (P<0.05).In terms of adenoid facial appearance,the internal and external treatment group was better than the traditional Chinese medicine internal treatment group (P<0.05).The size of adenoids was positively correlated with the total score of symptoms (P<0.05),and the total score of symptoms was positively correlated with the total score of quality of life (P<0.05).Conclusion:Sanjie Huatan Shuoxian decoction combined with acupoint massage and internal and external treatment can improve the clinical efficacy of children with adenoid hypertrophy and significantly improve the quality of life of the children.Its therapeutic effect is better than that of the western medicine group.
