微创INTERTAN髓内钉与PFNA-Ⅱ髓内钉治疗老年股骨粗隆间骨折的临床疗效分析

Analysis of clinical efficacy of minimally invasive INTERTAN intramedullary nail and PFNA-Ⅱintramedullary nail in the treatment of intertrochanteric fractures of the femur

目的观察PFNA-Ⅱ髓内钉与微创INTERTAN髓内钉对老年股骨粗隆间骨折的临床疗效。方法66例老年股骨粗隆间骨折患者,根据治疗方法不同分为微创组和基础组,每组33例。微创组患者采用INTERTAN髓内钉固定,基础组患者采用PFNA-Ⅱ髓内钉固定。比较两组手术相关指标、并发症发生情况及髋关节恢复情况。结果微创组手术出血量、术后输血量均少于基础组,手术时间、负重时间均短于基础组,疼痛评分低于基础组,差异具有统计学意义(P<0.05)。微创组患者并发症发生率为3.03%,明显低于基础组的18.18%,差异具有统计学意义(P<0.05)。微创组髋关节恢复优良率为93.94%,明显高于基础组的72.73%,差异具有统计学意义(P<0.05)。结论INTERTAN髓内钉固定效果好,可改善老年股骨粗隆间骨折患者预后,并发症少,且髋关节恢复优良率高。

Objective To observe the clinical efficacy of minimally invasive INTERTAN intramedullary nail and PFNA-Ⅱintramedullary nail in the treatment of intertrochanteric fractures of the femur.Methods A total of 66 elderly patients with intertrochanteric fractures of the femur were divided into minimally invasive group and basic group,with 33 cases in each group.The minimally invasive group was treated with minimally invasive INTERTAN intramedullary nail,and basic group was treated with PFNA-Ⅱintramedullary nail.The operationrelated indicators,occurrence of complications and hip joint recovery were compared between the two groups.Results The operative blood loss,postoperative blood transfusion were less than those of basic group,operation time and weight-bearing time of minimally invasive group were shorter than those of basic group,and pain score was lower than that of basic group.All the difference was statistically significant(P<0.05).The incidence of complications of the minimally invasive group was 3.03%,which was obviously lower than 18.18%of basic group,and the difference was statistically significant(P<0.05).The excellent-good rate 93.94%of hip joint recovery of minimally invasive group was obviously higher than 72.73%of basic group,and the difference was statistically significant(P<0.05).Conclusion The INTERTAN intramedullary nail has a good fixation effect and can improve the prognosis of elderly patients with intertrochanteric fractures of the femur,with fewer complications and a high excellent-good rate of hip joint recovery.
