克罗米芬联合小剂量尿促治疗无排卵性功血的疗效评价

Efficacy evaluation of clomiphene combined with low-dose human menopausal gonadotropin inthe treatment of anovulatory dysfunctional uterine bleeding

目的探讨无排卵性功能失调性子宫出血(功血)患者采用克罗米芬联合小剂量尿促性素(尿促)治疗的疗效。方法86例无排卵性功血患者,按1∶1比例分为观察组和对照组,每组43例。对照组患者采用雌孕激素治疗,观察组患者采用克罗米芬联合小剂量尿促性素治疗。比较两组患者临床疗效、性激素水平、临床指标。结果观察组患者总有效率为95.35%,明显高于对照组的81.40%,差异具有统计学意义(P<0.05)。治疗前,两组患者促黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、孕酮(P)、睾酮(T)水平比较差异无统计学意义(P>0.05);治疗后,观察组患者LH(7.81±1.41)IU/L、FSH(17.51±0.94)IU/L、E2(464.38±13.84)pmol/L、P(46.08±3.81)nmol/L、T(2.81±0.14)nmol/L均高于对照组的(6.31±1.52)IU/L、(15.02±0.98)IU/L、(445.28±15.02)pmol/L、(41.38±4.02)nmol/L、(2.64±0.13)nmol/L,差异具有统计学意义(P<0.05)。观察组患者内分泌功能复常时间为(31.52±5.64)d、治疗时间为(42.28±7.42)d,均短于对照组的(40.95±5.25)、(51.58±6.89)d,差异具有统计学意义(t=8.0252、6.0228,P=0.0000、0.0000<0.05)。结论克罗米芬联合小剂量尿促性素在无排卵性功血治疗中效果确切,可改善性激素水平,提高临床疗效,缩短治疗时间,值得借鉴。

Objective To discuss the effect of clomiphene combined with low-dose human menopausal gonadotropin in the treatment of anovulatory dysfunctional uterine bleeding.Methods A total of 86 patients with anovulatory dysfunctional uterine bleeding were divided into observation group and control group,with 43 cases in each group.The control group was treated with estrogen and progesterone,and the observation group was treated with clomiphene combined with low-dose human menopausal gonadotropin.The clinical efficacy,sex hormones level and clinical indicators were compared between the two groups.Results The total effective rate of the observation group was 95.35%,which was obviously higher than 81.40%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in luteinizing hormone(LH),follicle stimulating hormone(FSH),estradiol(E2),progesterone(P),testosterone(T)between the two groups(P>0.05).After treatment,the LH(7.81±1.41)IU/L,FSH(17.51±0.94)IU/L,E2(464.38±13.84)pmol/L,P(46.08±3.81)nmol/L and T(2.81±0.14)nmol/L of the observation group were all higher than(6.31±1.52)IU/L,(15.02±0.98)IU/L,(445.28±15.02)pmol/L,(41.38±4.02)nmol/L and(2.64±0.13)nmol/L of the control group,and the difference was statistically significant(P<0.05).The endocrine function recovery time(31.52±5.64)d and treatment time(42.28±7.42)d of the observation group were all shorter than(40.95±5.25)and(51.58±6.89)d of the control group,and the difference was statistically significant(t=8.0252,6.0228;P=0.0000,0.0000<0.05).Conclusion Combination of clomiphene and low-dose human menopausal gonadotropin has a definite effect in the treatment of anovulatory dysfunctional uterine bleeding,which can improve the level of sex hormones,improve the clinical efficacy,and shorten the treatment time.It is worth learning.
