张士芳应用当归拈痛汤加减治疗湿疹的临床经验分析

Analysis of Zhang Shifang's clinical experience in treating eczema with modified Danggui Niantong decoction

湿疹是发生于表皮或真皮浅层的一种迟发型变态反应性皮肤炎症疾病,皮损具有多形性、对称性、瘙痒、渗出倾向以及易反复发作等特点。西医多采用糖皮质激素外用法治疗湿疹,但部分患者疗效欠佳,且长时间应用可能出现较多不良反应。张士芳主任是航天中心医院原副院长,2018年北京市中医药管理局“薪火传承3+3工程”基层老中医。张士芳从医60余年,临床遣方用药常将时方与经方相结合,对湿疹的诊疗有独到经验。本文旨在总结张士芳老中医从风、湿、热邪出发运用当归拈痛汤治疗湿疹的临床思路及经验,分析中医如何论治该病,以期能提高临床上湿疹患者的生活质量、降低临床复发率。

Eczema is a delayed-type allergic skin inflammatory disease that occurs in the epidermis or the superficial layer of the dermis.The skin lesions have pleomorphic,symmetrical,itching,exudative tendency and repeated attacks.Western medicine mostly uses glucocorticoids for external treatment,but some patients have poor curative effect,and long-term use may have many adverse reactions.Zhang Shifang director is the former vice-president of the aerospace center hospital.He is a veteran traditional Chinese medicine expert in the"Xinhuo Inheritance 3+3 Project"of Beijing Administration of Traditional Chinese Medicine in 2018.Zhang Shifang has been engaged in medical work for more than 60 years.He often combines modern prescriptions with traditional prescriptions when using drugs in clinical practice especially in the field of skin diseases,and has unique experience in the diagnosis and treatment of eczema.This paper aims to summarize Zhang Shifang's clinical thinking and experience in treating eczema with Danggui Niantong decoction from wind,dampness and heat,and analyze how to treat the disease by Chinese medicine in order to improve the quality of life of patients with eczema in clinic and reduce the clinical recurrence rate.
