摘要
目的研究关节镜下空心钉固定治疗急性移位单纯性肱骨大结节骨折患者的临床效果。方法回顾性选取100例急性移位单纯性肱骨大结节骨折患者,依据手术方法将其分为关节镜下空心钉固定组(n=50,关节镜下空心钉固定)与切开复位钢板内固定组(n=50,切开复位钢板内固定)。比较两组的临床效果。结果关节镜下空心钉固定组的切口长度、手术时间、术中出血量、住院时间、骨折愈合时间及术后并发症总发生率均优于切开复位钢板内固定组(P<0.05)。手术后,两组的患侧肩关节前屈上举、外展、外旋活动度、Constant评分均升高,内旋活动度、VAS评分均降低,且关节镜下空心钉固定组优于切开复位钢板内固定组(P<0.05)。结论相较于切开复位钢板内固定,关节镜下空心钉固定治疗急性移位单纯性肱骨大结节骨折患者的临床效果更显著。
Objective To study the clinical effect of arthroscopic cannulated screw fixation in the treatment of patients with acute displaced simple humeral greater tuberosity fracture.Methods A total of 100 patients with acute displaced simple humeral greater tuberosity fracture were selected retrospectively,and the patients were divided into arthroscopic cannulated screw fixation group(n=50,arthroscopic cannulated screw fixation)and open reduction and plate internal fixation group(n=50,open reduction and plate internal fixation)by the operation method.The clinical effects of the two groups were compared.Results The incision length,operation time,intraoperative blood loss volume,hospital stay,fracture healing time and the total incidence of postoperative complications of the arthroscopic cannulated screw fixation group were better than those of the open reduction and plate internal fixation group(P<0.05).After operation,the range of motion of affected shoulder joint flexion,abduction and external rotation and Constant score of the two groups increased,while the internal rotation range of motion and VAS score decreased,and those of the arthroscopic cannulated screw fixation group were better than the open reduction and plate internal fixation group(P<0.05).Conclusion Compared with open reduction and plate internal fixation,arthroscopic cannulated screw fixation in the treatment of patients with acute displaced simple humeral greater tuberosity fracture has more significant clinical effect.
