针灸联合散瘀止痛散治疗腰椎间盘突出症的临床疗效

Clinical effect of acupuncture combined with Sanyu Zhitong powder on lumbar disc herniation

目的探讨针灸联合散瘀止痛散治疗腰椎间盘突出症的临床疗效。方法选取南华大学附属长沙中心医院2018—2019年收治的腰椎间盘突出症患者87例,按照数字奇偶法分为对照组42例和观察组45例。2组均予以常规牵引、推拿、防寒保暖,对照组加用针灸治疗,观察组加用针灸联合散瘀止痛散治疗。比较2组腰腿疼痛程度、腰椎功能,治疗前后血清炎性因子水平,包括肿瘤坏死因子α(TNF-α)、白介素1β(IL-1β)及基质金属蛋白酶3(MMP-3)。结果治疗前2组视觉模拟评分法(VAS)评分、日本骨科学会(JOA)评分比较,差异无统计学意义(P>0.05);治疗后观察组VAS评分低于对照组,JOA评分高于对照组(P<0.05)。治疗前2组血清TNF-α、IL-1β、MMP-3水平比较,差异无统计学意义(P>0.05);治疗后观察组血清TNF-α、IL-1β、MMP-3水平低于对照组(P<0.05)。结论针灸联合散瘀止痛散治疗腰椎间盘突出症的临床疗效确切,可减轻患者腰腿疼痛程度,促使腰椎功能恢复,明显缓解机体炎性反应。

Objective To investigate the clinical effect of acupuncture combined with Sanyu Zhitong powder on lumbar disc herniation.Methods A total of 87 cases of patients with lumbar disc herniation were selected from 2018 to 2019 in Changsha Central Hospital Affiliated to Nanhua University,which were divided into control group(42 cases)and observation group(45 cases)according to digital parity method.Both groups were given conventional traction,massage,cold prevention and warmth,the control group was given acupuncture treatment,and the observation group was given acupuncture combined with Sanyu Zhitong powder.The degree of lumbocrural pain,lumbar function,serum inflammatory factors including tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and matrix metalloproteinase-3(MMP-3)before and after treatment were compared between the two groups.Results Before treatment,there was no significant difference in visual analogue scale(VAS)score and Japanese Orthopaedic Association(JOA)score between the two groups(P>0.05);after treatment,the VAS score of the observation group was lower than that of the control group,and the JOA score was higher than that of the control group(P<0.05).Before treatment,there was no significant difference in serum TNF-α,IL-1β and MMP-3 levels between the two groups(P>0.05);after treatment,the serum TNF-α,IL-1β and MMP-3 levels in the observation group were lower than those in the control group(P<0.05).Conclusion Acupuncture combined with Sanyu Zhitong powder in the treatment of lumbar disc herniation has a definite clinical effect.It can reduce the pain degree of patients’ waist and legs,promote the recovery of lumbar function,significantly relieve the inflammatory reaction of the body.
